# Assess eligibility for bariatric surgery in adolescents with obesity

## STEP 1 — Gather Information
Collect age, Tanner stage, BMI (kg/m2), obesity-related comorbidities (T2DM, hypertension, sleep apnea, NAFLD, etc.), documentation of ≥6-month intensive lifestyle modification program adherence, psychological evaluation results, and confirmation of access to a pediatric bariatric surgery center of excellence with experienced surgeon and multidisciplinary team.

## STEP 2 — Rule In / Rule Out
Is the patient Tanner stage 4-5 AND (BMI ≥40 kg/m2 OR [BMI ≥35 kg/m2 AND presence of significant comorbidity such as T2DM, moderate-to-severe sleep apnea, hypertension, or NAFLD])?  
- If **YES**, proceed to STEP 3.  
- If **NO**, rule out bariatric surgery eligibility; continue intensive lifestyle modification and reassess in 3-6 months.

## STEP 3 — Classify or Stratify
Evaluate: (1) documented compliance with lifestyle program (diet, activity, behavioral); (2) psychological evaluation confirming stability and competence of family unit, no untreated psychiatric illness; (3) patient demonstrates ability to adhere to healthy dietary and activity habits; (4) access to experienced surgeon at a pediatric bariatric surgery center of excellence capable of long-term follow-up.  
If **all four** criteria are met, classify as **eligible**; if any criterion is unmet, classify as **not eligible**.

## STEP 4 — Decide
If eligible, refer for bariatric surgery preoperative assessment and shared decision-making with family.  
If not eligible, address specific deficits (e.g., enhance lifestyle program adherence, obtain psychological clearance, treat substance abuse) and re-evaluate after optimization.

## Clinical Guardrails / Mimics / Pitfalls
Do not offer surgery to prepubertal patients (Tanner <4), those with untreated psychiatric illness, active substance abuse, inability to adhere to lifestyle changes, or lack of access to an experienced surgeon and multidisciplinary team; avoid surgery in pregnancy or planned pregnancy within 2 years.

A 16-year-old female with Tanner 5 pubertal development, BMI 42 kg/m2, hypertension, completed a 9-month supervised lifestyle program with documented adherence, stable mood on psychological evaluation, no substance use, and referred to a center with an experienced bariatric surgeon → eligible for surgery assessment.
