## STEP 1 — Gather Information
Collect serum creatinine, patient weight, age, and sex to estimate creatinine clearance (or use measured eGFR); confirm hypercalcemia of malignancy diagnosis; note the planned bisphosphonate (zoledronic acid or pamidronate).

## STEP 2 — Rule In / Rule Out
If estimated creatinine clearance <60 mL/min, rule in renal insufficiency and proceed to dose adjustment; if ≥60 mL/min, rule out renal insufficiency and use standard infusion.
**Decision:** Administer standard infusion (zoledronic acid over 15 minutes or pamidronate over 2 hours) when CrCl ≥60 mL/min; otherwise continue to dose adjustment.
## STEP 3 — Classify or Stratify
For zoledronic acid, extend infusion to 30‑60 minutes; for pamidronate, extend infusion to 2‑24 hours (choose duration based on clinical judgment and institutional protocols).
