## Failure Modes
### Confirmation Bias
- **Symptom**: All sources support your initial hunch; feeling confident after 3 sources
- **Detection**: If you haven't found ANY contradictory evidence, you're cherry-picking
- **Fix**: Before synthesis, search "[conclusion] problems" or "[alternative] advantages"; steel-man opposing view
### Analysis Paralysis
- **Symptom**: Research extends past time-box; finding "just one more source"; perfectionist language
- **Detection**: If you've been researching for 2x your time-box without synthesizing
- **Fix**: Force synthesis at time boundary; note gaps as "future research"; incomplete answer beats perfect never
### Source Quality Blindness
- **Symptom**: Treating blog posts and peer-reviewed papers equally; no source tier labels
- **Detection**: If conclusion relies primarily on T3 sources without acknowledging it
- **Fix**: Tier-label every source (T1/T2/T3); flag low-confidence if conclusion rests on T3
### Scope Creep Rabbit Hole
- **Symptom**: Research question keeps expanding; interesting tangents become main focus
- **Detection**: If current research doesn't map back to original decision context
- **Fix**: Re-read Step 1 boundaries; note tangents as "future research"; return to scope
### Synthesis Avoidance
- **Symptom**: Endless summarizing without conclusions; "the sources say X, Y, Z" without "therefore"
- **Detection**: If your output is longer than sources combined or lacks clear recommendation
- **Fix**: Force 3-paragraph synthesis maximum; extract conclusion sources didn't individually state
