## Workflow
`plan` is chat-only. The Intent-Ready Gate separates each discoverable fact
from each user-owned decision before bounded grounding.
Use decision dependencies to ask a parent decision separately or group
independent decisions from the same frontier. Use no fixed question limit and
include a recommended answer for every decision.
Use the Proposal-Ready Gate after grounding returns.
Settle Goal, Scope, Approach, Validation, and Plan Strategy.

## Output
Return `clarifying`, `blocked`, `proposal-ready`, or `handoff`.
