# Nutrition Science: Why Nutrition Science Is Hard, Confounding and Study Design, and the Measurement Problem
> **Part 1 of 6** of the *Food and Nutrition Science* reference (plugin `food-and-nutrition-science`), covering §0–§3. Sibling skills: `nutrition-macronutrients-vitamins-minerals-and-supplements` (§4–§10), `nutrition-digestion-energy-balance-appetite-and-food-processing` (§11–§18), `nutrition-dietary-patterns-weight-regulation-and-life-stages` (§19–§23), `nutrition-live-evidence-and-reading-a-claim` (§24–§25), `nutrition-reference` (§26–§30). Section numbers are shared across the set; a reference written as §N → `skill` points into that sibling skill.
>
> **Currency:** The biochemistry is settled; the applied layer is contested. Two areas are live. See §24 → `nutrition-live-evidence-and-reading-a-claim` for ultra-processed foods, and the 2025-2030 US Dietary Guidelines.
> **⚠️ A field where the biochemistry is solid, the epidemiology is weak, and the public
> discourse is almost entirely disconnected from both.** ⚠️ **The gap between what
> nutrition science can actually establish and the confidence of nutrition headlines is
> wider than in almost any other discipline** — **and understanding WHY (§1–§3) matters
> more than memorizing any particular finding.**
>
> **Complements a cooking/cleaning reference (food safety and the chemistry of cooking),
> an exercise physiology reference (§12's energy expenditure), and a psychology reference
> (§1's replication context).**
>
> **⚠️ GOTCHA** boxes mark the claims that outrun their evidence.
>
> **⚠️ Two things stated up front.** ⚠️ **First, this is a reference on the SCIENCE — it is
> not personalized advice, and individual circumstances (medication, pregnancy, kidney or
> liver disease, absorption disorders) change the answers substantially.** ⚠️ **Second,
> §22 → `nutrition-dietary-patterns-weight-regulation-and-life-stages` covers disordered eating and is placed BEFORE the diet content deliberately,
> because nutrition information is not neutral for everyone who reads it.**
>
> **The three ideas that organize this document:**
> 1. **⚠️ Almost all nutrition epidemiology is confounded, and the confounding runs the
> same direction every time** (§2). **Healthy-user bias means the people who follow any
> dietary advice differ systematically from those who don't.**
> 2. **⚠️ Dietary PATTERNS are better supported than single nutrients** (§19 → `nutrition-dietary-patterns-weight-regulation-and-life-stages`). **Decades of
> single-nutrient reductionism produced reversals; the whole-diet literature has held
> up better.**
> 3. **⚠️ The measurement instrument is the field's foundational weakness** (§3). **Most
> large studies rest on people remembering what they ate, and that error is not random.**

# PART I — EPISTEMICS
## §1. ⚠️ Why Nutrition Science Is Hard
```
⚠️ EVERYONE IS ALWAYS IN THE EXPOSURE. There is no unexposed control —
you cannot have a no-diet group
⚠️ EFFECTS ARE SMALL AND SLOW. Chronic disease develops over decades;
RCTs run for months
⚠️ SUBSTITUTION IS UNAVOIDABLE. Eating less of X means eating more of
something else — ⚠️ "is X bad" is meaningless without "instead of what"
⚠️ COMPLIANCE DECAYS. Long dietary RCTs see arms converge, which
biases toward the null
⚠️ MEASUREMENT IS SELF-REPORT (§3)
⚠️ FUNDING AND IDEOLOGY. Industry funding is pervasive; so is
ideological commitment among researchers. ⚠️ BOTH distort, and
dismissing an argument by its funding is not the same as
refuting it (§24.1)
```
**⚠️ The reliability gradient:**
```
⚠️ ROBUST ⚠️ Nutrient deficiency diseases and their cures (scurvy,
beriberi, pellagra, rickets, iodine deficiency) — ⚠️ these are the
field's genuine triumphs, established by intervention with
dramatic effects · ⚠️ energy balance thermodynamics · basic
biochemistry and requirements · trans fat harm · ⚠️ folate
fortification preventing neural tube defects
⚠️ REASONABLY SUPPORTED Mediterranean-style patterns · sodium and
blood pressure · fibre and colorectal outcomes · sugar-sweetened
beverages and metabolic risk
⚠️ CONTESTED ⚠️ saturated fat's magnitude of effect · UPF causality
(§24.1) · optimal macronutrient ratios · red meat · most
supplement claims beyond deficiency
⚠️ WEAK OR OVERTURNED ⚠️ dietary cholesterol as a major driver of
serum cholesterol for most people · ⚠️ the low-fat orthodoxy of
1980-2000 · ⚠️ most single-antioxidant claims · detox concepts ·
⚠️ "alkaline diet" · blood-type diets · most "superfood" claims
```
---
## §2. ⚠️ Confounding and Study Design
```
⚠️ HEALTHY USER BIAS — THE central problem. People who follow dietary
advice also exercise more, smoke less, drink less, sleep better,
are wealthier and have better healthcare access. ⚠️ Statistical
adjustment cannot fully remove what it cannot measure
⚠️ REVERSE CAUSATION early illness changes appetite and diet
⚠️ CONFOUNDING BY INDICATION people change diet BECAUSE of a diagnosis
⚠️ THE SUBSTITUTION PROBLEM (§1)
⚠️ RESIDUAL CONFOUNDING what's left after adjustment, and it is
the reason a hazard ratio of 1.2 in nutrition epidemiology
should not move you much
```
**⚠️ Study designs, ranked by what they can support:**
```
⚠️ RCT with HARD OUTCOMES the gold standard, and ⚠️ rare, expensive
and short in nutrition. PREDIMED is the notable example (⚠️ and was
retracted and republished after randomization irregularities —
a useful cautionary case)
⚠️ METABOLIC WARD studies ⚠️ tight control, tiny n, short duration,
highly artificial. Good for MECHANISM, weak for real-world effect
COHORT STUDIES ⚠️ the bulk of the literature. Hypothesis-generating
CASE-CONTROL recall bias on top of everything else
⚠️ MENDELIAN RANDOMIZATION ⚠️ genuinely useful — uses genetic
variants as unconfounded proxies for lifelong exposure.
⚠️ Its assumptions (no pleiotropy) are often unverifiable
```
> **⚠️ GOTCHA — the vitamin E case is the one to remember.** ⚠️ **Observational studies
> consistently associated higher vitamin E intake with lower cardiovascular risk;
> large RCTs found no benefit and some signals of harm.** **⚠️ The same pattern repeated
> for beta-carotene (where a trial in smokers found INCREASED lung cancer) and for hormone
> replacement therapy in a different field.** **⚠️ When a strong observational association
> is tested by randomization, it frequently disappears — and that is the base rate you
> should carry into any new observational claim.**
---
## §3. ⚠️ The Measurement Problem
**⚠️ The field's foundational weakness, and it is rarely stated plainly in press coverage.**
```
⚠️ FFQ (food frequency questionnaire) — ⚠️ the workhorse instrument.
Asks people to recall typical intake over months or a year.
⚠️ Correlations with actual intake are moderate at best
⚠️ 24-HOUR RECALL better for a day, ⚠️ but a day is not a diet
⚠️ FOOD DIARIES ⚠️ the act of recording CHANGES what people eat
⚠️ UNDER-REPORTING IS SYSTEMATIC, NOT RANDOM. ⚠️ Energy intake is
commonly under-reported, and under-reporting correlates with
body weight and with social desirability — ⚠️ which means the
error is correlated with the OUTCOME. That is the worst kind
⚠️ BIOMARKERS doubly labelled water for energy, urinary nitrogen for
protein, urinary sodium. ⚠️ Objective, expensive, and they
consistently show self-report is worse than assumed
⚠️ FOOD COMPOSITION TABLES ⚠️ another error layer — actual foods vary
by variety, soil, season, ripeness and preparation
```
**⚠️ The consequence**: ⚠️ **measurement error attenuates real effects toward the null AND,
when the error is differential, can manufacture associations that don't exist.**
**⚠️ Some methodologists argue memory-based dietary data are unfit for estimating
population intake at all** — **that is a minority position, and it is a serious one.**
