# Cohere Incident Runbook
## Overview
Rapid incident response procedures for Cohere API v2 outages. Covers triage, mitigation, communication, and postmortem for Chat, Embed, Rerank, and Classify endpoints.
## Prerequisites
- Access to [status.cohere.com](https://status.cohere.com)
- kubectl access to production cluster
- Prometheus/Grafana access
- PagerDuty/Slack communication channels
## Severity Levels

## Instructions
Classify the affected model/endpoint and data sensitivity, stabilize outbound
traffic with the approved fallback or circuit breaker, preserve redacted
correlation/timing evidence, and communicate through the defined incident
cadence. Restore the smallest safe change, then verify behavior and data
integrity before closing or replaying queued work.
## Error Handling
| Condition | Response |
|---|---|
| Authentication/key compromise | Stop the affected workload, rotate through the secret owner, and verify scoped recovery. |
| Provider outage or severe 5xx | Use the approved fallback/circuit breaker and preserve request state for safe retry. |
| Rate/timeout exhaustion | Defer through bounded retry policy; do not amplify traffic. |
| Unsafe output or data-handling concern | Disable affected feature path and engage the safety/data owner. |
## Examples
During a provider outage, trip the model circuit breaker, keep queued work with
idempotency keys, notify stakeholders using the template, and test recovery on
a small staging request before reopening. If a key is exposed, rotate it and
validate logs/configuration before processing any previously failed request.
