# Design Adventure Trip Risk Assessment
Design a systematic risk assessment for adventure travel activities — identifying hazards, evaluating risk factors (likelihood × severity), establishing go/no-go criteria, and planning emergency response — to enable informed decision-making before high-risk activities.
## Why This Is Best Practice
**Adopted by:** The National Outdoor Leadership School (NOLS) risk management model is the industry standard for outdoor education and adventure guiding programs globally. The Adventure Travel Trade Association (ATTA) uses systematic risk assessment as a requirement for member operators. The US Army's Risk Management process (FM 5-19) is the formal military standard; adapted versions are used in commercial adventure operations. Wilderness medicine research (WMS) provides the evidence base for medical risk factors at altitude, underwater, and in remote environments.
**Impact:** Adventure travel accidents are the leading cause of injury death among travelers, second only to road traffic accidents. Most are preventable: analysis of mountaineering fatalities (Williamson & Weiss, 2012) found that 60%+ involved errors in risk assessment or go/no-go decision-making — proceeding despite observable hazard indicators. Systematic pre-activity risk assessment documents hazards, creates explicit criteria for turning back, and plans emergency response before the emotional commitment of the activity makes objective judgment difficult.
**Why best:** Pre-established, written go/no-go criteria and emergency plans made when calm outperform in-moment judgment, which is degraded by sunk cost, summit fever, and group social pressure at exactly the point risk is highest.
## Steps
### 1. Identify and categorize hazards
List all hazards for the planned activity — both objective (environmental, fixed) and subjective (human factors):
**Objective hazards (environmental):**
- Weather: rain, wind, temperature extremes, lightning, fog (for navigation)
- Terrain: avalanche zones, rockfall, swift water, exposure (height), depth/currents underwater
- Altitude: altitude sickness risk above 2,500 meters; AMS, HACE, HAPE at higher elevations
- Wildlife: animal attack risk (appropriate to region and activity)
- Natural events: flash floods, tidal changes, volcanic activity
**Subjective hazards (human):**
- Fitness and experience: does every participant have the physical capacity and skill for this activity?
- Equipment: is all equipment appropriate, functional, and properly fitted?
- Judgment under fatigue: decision-making deteriorates significantly with sleep deprivation and physical exhaustion
- Group dynamics: social pressure to continue despite safety concerns ("summit fever"); hierarchy that prevents junior members from raising concerns
**Risk = Likelihood × Severity matrix:**

High-likelihood, high-severity hazards (e.g., crossing a flooded river in a flash-flood region during rain) are a no-go condition.
### 2. Assess participant fitness and skill match
The primary source of adventure travel accidents: participants whose fitness or skill level doesn't match the activity requirements:
- **Fitness assessment:** does the planned activity have a physically demanding component (altitude, distance, current, sustained exertion)? Assess each participant honestly.
- **Skill assessment:** is specialized training required (belaying, dive certification, swift-water skills, navigation)? Verify certifications and recency of practice; a certification earned 5 years ago may not reflect current skill.
- **Medical screening:** is any participant taking medications that affect judgment, balance, or physical capacity? Are there conditions that increase altitude, exertion, or depth risk (heart conditions, COPD, ear problems for diving)?
**Self-assessment honesty:** adventure travel deaths disproportionately involve overestimation of personal capability; design the assessment to be completed when calm and not in the moment of group momentum.
### 3. Establish pre-activity go/no-go criteria
Go/no-go criteria must be established before the activity begins, when judgment is uncompromised by sunk cost and group momentum:
**Format:** "We will not proceed if [specific condition]"
Examples:
- Mountaineering: "We will not summit if wind speed exceeds 40 km/h, if cloud ceiling drops below the summit line, if any team member shows symptoms of AMS (headache, nausea, ataxia), or if we cannot reach [specific point] by [specific time]"
- Scuba diving: "We will not dive if visibility drops below 5 meters, if any diver reports ear equalization difficulty, if current exceeds [X] knots, or if the emergency oxygen kit is not accessible"
- River crossing: "We will not cross if the water reaches above knee height, if any participant cannot safely turn around, or if water clarity prevents seeing the bottom"
**Why criteria must be pre-established:** in the moment, summit fever and group social pressure produce poor judgment; a written checklist made earlier overrides in-moment emotion.
### 4. Plan emergency response before departure
Emergency response planning before departure, when calm, is essential:
- **Communication plan:** does every participant know the emergency contact and check-in schedule? Is there communication equipment (satellite phone, PLB, VHF radio) appropriate to the environment?
- **Evacuation plan:** what is the evacuation route? What is the nearest medical facility? What is the transportation time and method?
- **Medical kit:** is a wilderness first aid kit present? Does someone have wilderness first aid certification? Is an EpiPen present if any participant has severe allergies?
- **Emergency contacts:** who are the emergency contacts? Have they been notified of the itinerary and expected return time?
**The trip float plan:**
- Leave with a reliable person: where you are going, by what route, your expected return, and at what point they should call emergency services
- Specific trigger: "If I have not returned by [date/time] and have not made contact by [time], call [emergency number] and report me as overdue"
### 5. Review the plan at key decision points during the activity
Risk assessment is not a one-time pre-departure exercise — it is an ongoing process:
- **Dawn assessment:** each morning of a multi-day activity, re-evaluate weather and conditions against go/no-go criteria
- **Pre-crux assessment:** before the most dangerous section (the summit push, the river crossing, the descent), pause and explicitly re-evaluate: are all go criteria still met?
- **Gut check:** when something feels wrong, name it explicitly and evaluate it against the pre-established criteria; the "something feels off" intuition is often pattern recognition detecting a hazard that hasn't been consciously identified
## Common Mistakes
- **Establishing go/no-go criteria only in retrospect (after the accident):** criteria must be established before the activity, when judgment is uninfluenced by sunk cost and summit fever; the "we're already here" fallacy is documented in every major study of mountaineering and adventure accidents.
- **Failing to include a turn-around time:** time-based criteria prevent overcommitment; "we turn back at 2pm regardless of progress" is the single most effective mountaineering safety rule
- **Ignoring team member concerns:** a team member who expresses hesitation or discomfort before a high-risk activity is providing important information; dismissing the concern because of group momentum is a documented accident precursor.
