## Hard guardrails
- Git operations are **read-only** (`log`, `diff`, `blame`, `show`, `ls-files`). Never commit, checkout, stash, or reset.
- Never modify a doc before the user approves the diff in Step 3.
- Additive edits only. Do not delete or reword existing sections unless the user explicitly asks; propose a "Superseded" note instead.
- Timestamps are ISO-8601 UTC with seconds: `YYYYMMDDTHHMMSSZ`.
- Every proposed change carries an evidence grade. Never promote a HYPOTHESIS into a doc as a fact.
- Verify each written file landed on disk before reporting success.
## Modes

User can override lookback with any phrase: "last 30 days", "since v2.3", "since the last SOP edit". "Since last doc edit" = `git log -1 --format=%cI -- <doc>`; fall back to file mtime.

## Step 1 — Deep recon
### 1a. Find existing SOP / lessons docs
Search case-insensitively, all text formats (`.md .txt .rst .adoc`), excluding `node_modules`, `vendor`, `.git`, build dirs.
Filename signals: `SOP`, `standard-operating`, `runbook`, `playbook`, `procedure`, `LESSONS`, `lessons-learned`, `learnings`, `postmortem`, `post-mortem`, `retro`, `incident`, `INCIDENT-*`, `RCA`, `CHANGELOG`, `DECISIONS`, `ADR`, `HOWTO`, `ops`.
Directory signals: `docs/`, `doc/`, `runbooks/`, `ops/`, `.github/`, `wiki/`, `notes/`, `handbook/`.
Content signals (grep when filenames are unhelpful): "lessons learned", "what went wrong", "root cause", "action items", "never again", "gotcha", "standard operating".
Also note governance files if present — `CLAUDE.md`, `AGENTS.md`, `PDDA.md`, `CONTRIBUTING.md`, `.claude/`, `.cursorrules` — **only to learn local conventions** (headings, tone, where docs live). The skill does not depend on any of them.
Classify each hit: **SOP** (how we do X), **LESSONS** (what we learned / what broke), **BOTH**, or **OTHER** (changelog, ADR — mine as a source, don't update).
### 1b. If none found → offer to scaffold
Ask before creating. Location rule:
1. If a `docs/` (or `doc/`) directory exists → `docs/SOP.md` and `docs/LESSONS.md`.
2. Else repo root → `SOP.md` and `LESSONS.md`.
3. If governance files specify a docs path, honor that instead and say so.

**Evidence grades** (use exactly these labels):
- **FACT** — directly observable in repo state or a committed artifact (diff, merged PR, incident file).
- **PATTERN** — same signal from ≥2 independent sources, or a recurrence of a known failure.
- **HYPOTHESIS** — inferred from one weak signal (commit message wording, a transcript remark, a TODO).
Emit a recon summary before proposing anything:

Rules:
- Quote existing lines being amended verbatim; show additions with `+`.
- HYPOTHESIS items go in a separate **"Low-confidence — confirm or drop"** section and are never applied without explicit user confirmation.
- End with a **"What I could not verify"** section — never leave it out. If empty, write "nothing".
- Then ask the confirmation question (Step 3). Do not apply yet.

## Step 3 — Confirm events, then apply
Ask, in this order, one message:
1. "Any specific event, incident, or decision from this period that must be captured that I didn't surface?" — if yes, mine it, grade it, add a proposal block, re-show.
2. "Approve all / approve by ID (e.g. C1 C3 C5) / edit / drop?"
Only after approval:
- Apply approved diffs additively. New entries go at the **top** of their section (newest first) unless the doc's existing convention is oldest-first — follow the doc.
- Write an audit record to `.sop/<YYYYMMDDTHHMMSSZ>/proposal.md` (the full Step 2 output) and update `.sop/INDEX.md` (one line per run: stamp, mode, window, approved IDs). Skip this only if the user says no.
- Re-read each modified file and confirm the new lines are present. Report: files changed, IDs applied, IDs dropped, anything unverified.
- Do **not** commit. Suggest a commit message; the user commits.

## Governance-layer neutrality
- If `CLAUDE.md` / `AGENTS.md` / `PDDA.md` define doc locations, headings, or a review gate — follow them, and say which rule you followed.
- If a governance layer forbids writing docs from an agent, stop at Step 2 and deliver the proposal file only.
- Never require a governance file to exist. A bare folder with a `.git` and a `README` is a fully supported input.
## Failure handling
- No git repo → mine file mtimes and any logs; grade everything HYPOTHESIS unless corroborated; say so.
- `gh` unavailable or unauthenticated → skip issues/PRs, note it under "could not verify".
- Window has zero events → say so plainly; offer Deep mode or a wider window. Do not invent candidates.
- Doc is huge (>1000 lines) → target sections by heading match; show only the touched section in the diff.
