Is the initial tT <12 nmol/L (≈350 ng/dL)? If ≥12 nmol/L, rule out hypogonadism. If <12 nmol/L, repeat tT on a separate day (morning fasting). If the repeat tT is ≥12 nmol/L, rule out hypogonadism; if both measurements are <12 nmol/L, proceed to diagnosis.

A 48‑year‑man reports reduced libido, morning erections, and fatigue. Initial tT at 09:00 h fasting is 10.8 nmol/L; repeat tT four weeks later is 11.2 nmol/L. Both values are <12 nmol/L and symptoms are present, so male hypogonadism is confirmed.
