## STEP 2 — Rule In / Rule Out

## STEP 3 — Classify or Stratify

If any of these factors are present → contraindicate bariatric surgery. If none are present → patient may be considered for surgical evaluation per section 4.11.

If any contraindication identified in steps 2‑3, recommend against bariatric surgery and initiate or intensify medical management (lifestyle, pharmacotherapy if appropriate, behavioral health). If no contraindications exist, proceed to assess surgical eligibility using BMI thresholds, comorbidity severity, psychological evaluation, and multidisciplinary team clearance per guideline 4.11.
