# Assess renal function before administering intravenous bisphosphonate
## STEP 1 — Gather Information
Collect serum creatinine, age, weight, sex, and race to calculate estimated glomerular filtration rate (eGFR) or creatinine clearance (CrCl) using CKD-EPI or Cockcroft-Gault; note current medications, volume status, and any acute kidney injury.
## STEP 2 — Rule In / Rule Out
Determine if eGFR/CrCl is available and not severely reduced (<30 mL/min/1.73m2); if unavailable or acute kidney injury suspected, delay bisphosphonate until renal function assessed.
## STEP 3 — Classify or Stratify
Stratify renal function: ≥90 mL/min/1.73m2 (normal), 60-89 (mild), 30-59 (moderate), <30 (severe) based on eGFR or CrCl.
## STEP 4 — Decide
- Normal/mild: administer standard IV bisphosphonate infusion (e.g., zoledronic acid over 15 min, pamidronate over 2-4 hours).
- Moderate: use prolonged infusion (zoledronic acid over 30-60 minutes, pamidronate over 2-24 hours) per dosing adjustments.
- Severe (<30): avoid IV bisphosphonate; consider denosumab or consult nephrology for dose adjustment.
## Clinical Guardrails / Mimics / Pitfalls
Do not administer IV bisphosphonate without prior renal function assessment; avoid relying on serum creatinine alone; monitor for post-infusion renal worsening; in volume depletion, rehydrate first; consider alternative agents if eGFR <30.
## Concrete Clinical Example
A 68-year-old woman with breast cancer and hypercalcemia (serum calcium 13.8 mg/dL) planned for zoledronic acid 4 mg. Serum creatinine 1.2 mg/dL, age 68, weight 70 kg, female, non-black yields eGFR ≈55 mL/min/1.73m2 (moderate impairment). Decision: administer zoledronic acid over 30-60 minutes and check renal function 48 hours post-infusion.
