# Against routine vitamin D testing in adults 50-74
## STEP 1 — Gather Information
Collect patient age (50-74 years), presence of symptoms suggestive of vitamin D deficiency (e.g., bone pain, proximal muscle weakness), known conditions that alter vitamin D metabolism (malabsorption, CKD, hyperparathyroidism, osteoporosis treatment), and any established indications for 25(OH)D testing (hypocalcemia, monitoring therapy). If none, proceed.
## STEP 2 — Rule In / Rule Out
Rule in if patient has an established indication for testing (symptoms of deficiency, malabsorption, CKD stage 4-5, hyperparathyroidism, osteoporosis treatment monitoring, or other specific clinical indication). Rule out if asymptomatic and no such indications.
## STEP 3 — Classify or Stratify
Classify as "no routine testing indicated" when patient is aged 50-74, asymptomatic, and lacks established indications for 25(OH)D testing. No further stratification needed.
## STEP 4 — Decide
Do not order a 25(OH)D test. Discuss empiric vitamin D supplementation only if patient meets criteria for supplementation per other guideline recommendations (e.g., high-risk prediabetes, pregnancy, age ≥75), not based on test results.
## Clinical Guardrails / Mimics / Pitfalls
Do not use 25(OH)D levels to guide supplementation in this age group; avoid testing for cost and equity reasons; beware of false-positive low results leading to unnecessary treatment; do not extrapolate to patients with malabsorption, CKD, or osteoporosis where testing may be warranted.
## Concrete Clinical Example
A 62-year-old man with no bone or muscle symptoms, normal renal function, and no history of osteoporosis asks whether he should have a vitamin D level checked. The clinician advises against routine testing, discusses diet and sun exposure, and notes that empiric supplementation is not indicated without specific risk factors.
