# Against routine vitamin D screening in dark complexion adults
## STEP 1 — Gather Information

Action: If the patient is an adult with dark complexion and lacks established indications for testing, proceed to Step 2.
## STEP 2 — Rule In / Rule Out
If any established indication for 25(OH)D testing is present (e.g., hypocalcemia, malabsorption, renal osteodystrophy), rule in: proceed to targeted testing per standard of care (outside this guideline). Otherwise, rule out routine screening and proceed to Step 3.
## STEP 3 — Classify or Stratify
Classify the patient as a candidate for empiric vitamin D supplementation only if they belong to a group with a guideline recommendation for empiric supplementation (age 1‑18 years, ≥75 years, pregnancy, or high‑risk prediabetes); otherwise classify as not indicated for empiric supplementation.
Action: If classified as a candidate for empiric supplementation, proceed to Step 4A; if not, proceed to Step 4B.
## STEP 4 — Decide
4A: For candidates for empiric supplementation, initiate age‑appropriate empiric vitamin D dosing (per guideline) without prior 25(OH)D testing.
4B: For those not meeting empiric supplementation criteria, do not order 25(OH)D testing and do not initiate empiric vitamin D supplementation; advise on dietary sources and safe sun exposure as needed.

Do not use race as a proxy for dark complexion; assess skin pigmentation directly. Do not extrapolate to patients with conditions that alter vitamin D metabolism (e.g., malabsorption, renal disease). Do not order 25(OH)D testing based solely on skin complexion. Do not initiate high‑dose intermittent vitamin D without a specific indication. Do not assume low 25(OH)D levels based on complexion alone.
