# Perform visual field testing when tumor abuts optic chiasm
## STEP 1 — Gather Information
Obtain a pituitary MRI (or CT if MRI contraindicated) and assess whether the tumor abuts or compresses the optic chiasm.
## STEP 2 — Rule In / Rule Out
If imaging shows tumor-chiasm abutment, proceed to visual field testing; if no abutment is seen, formal visual field testing is not required per this guideline (though clinical judgment may still warrant it).
## STEP 3 — Classify or Stratify
Perform automated perimetry (e.g., Humphrey 24‑2 or 30‑2) and classify the visual field as normal or abnormal (e.g., bitemporal hemianopia, unilateral defect, or diffuse loss).
## STEP 4 — Decide
If the visual field is abnormal, expedite neurosurgical referral for decompression; if normal, schedule routine clinical and imaging follow‑up per acromegaly management.
