# Apply statin dose adjustments in chronic kidney disease except for atorvastatin and fluvastatin
## STEP 1 — Gather Information
Collect CKD stage (eGFR), current statin name and dose, concomitant medications, and presence of atorvastatin or fluvastatin; then determine if the statin requires dose adjustment based on CKD stage and statin metabolism.
## STEP 2 — Rule In / Rule Out
Is the statin atorvastatin or fluvastatin? If yes, no dose adjustment is needed (rule out need for adjustment); if no, proceed to assess CKD stage for dose adjustment.
## STEP 3 — Classify or Stratify
Classify CKD stage: stages 1-2 (eGFR ≥60), stage 3a (45-59), stage 3b (30-44), stage 4 (15-29), stage 5 (<15) or dialysis; for statins requiring adjustment (pitavastatin, pravastatin, rosuvastatin, lovastatin, simvastatin), assign dose reduction per CKD stage.
## STEP 4 — Decide
Reduce statin dose per guideline: rosuvastatin ≤10 mg daily in CKD stage 3, avoid >10 mg; pravastatin ≤20 mg; pitavastatin ≤2 mg; lovastatin ≤20 mg; simvastatin ≤10 mg; atorvastatin and fluvastatin no adjustment; if already at or below these doses, continue current regimen.

Do not dose adjust atorvastatin or fluvastatin; avoid high-intensity statins in advanced CKD (stage 4-5); monitor for myopathy, especially with simvastatin >10 mg in CKD stage 3 or worse; avoid rosuvastatin >10 mg in CKD; consider drug interactions (e.g., cyclosporine, gemfibrozil); do not initiate statin in dialysis unless clear cardiovascular benefit outweighs risk.
## Concrete Clinical Example
A 65-year-old with type 2 diabetes and CKD stage 3b (eGFR 35 mL/min/1.73m²) is on simvastatin 20 mg daily; since simvastatin requires dose adjustment and CKD stage 3b, reduce to simvastatin 10 mg daily and monitor for muscle symptoms.
