# Avoid statin use in pregnancy or when trying to become pregnant
## STEP 1 — Gather Information
Assess pregnancy status, intention to conceive, last menstrual period, and contraceptive use in females of childbearing potential presenting for lipid therapy.
## STEP 2 — Rule In / Rule Out
Is the patient pregnant or actively trying to become pregnant? If yes, proceed to Step 4 (avoid statin); if no, proceed to Step 3.
## STEP 3 — Classify or Stratify
Evaluate lipid-lowering indication per guideline (e.g., LDL‑C >70 mg/dL, ASCVD risk, risk‑enhancing factors) to determine if statin therapy is otherwise indicated.
## STEP 4 — Decide
If pregnant or trying to conceive, do not initiate statin; emphasize lifestyle modification and postpone pharmacologic lipid therapy until after pregnancy. If not pregnant and statin is indicated, initiate appropriate statin therapy.
## Clinical Guardrails / Mimics / Pitfalls
Do not rely on statins for lipid control in pregnancy due to teratogenic risk (FDA Category X); avoid in lactation unless benefit outweighs risk; do not mistake elevated lipids in pregnancy as requiring statin therapy.
