# Avoid carbohydrate‑containing oral fluids preoperatively in patients with diabetes undergoing surgery
## STEP 1 — Gather Information
Identify adult patients with type 1, type 2, or other diabetes scheduled for elective surgery; review preoperative fluid orders for carbohydrate‑containing drinks (e.g., juice, sports drinks, carbohydrate‑rich clear liquids).
## STEP 2 — Rule In / Rule Out
Determine whether the planned oral fluids contain carbohydrates; if yes, rule in need to avoid; if only noncaloric fluids (water, black coffee, tea) are planned, rule out need for change.
## STEP 3 — Classify or Stratify
Classify the patient as having diabetes undergoing elective surgery and requiring avoidance of preoperative carbohydrate‑containing oral fluids; proceed to decision.
## STEP 4 — Decide
Do not administer carbohydrate‑containing oral fluids; prescribe noncaloric clear fluids (water, black coffee, tea) up to 2 hours before surgery per standard NPO guidelines.
## Clinical Guardrails / Mimics / Pitfalls
Do not confuse with carbohydrate loading protocols used in nondiabetic patients; avoid giving carbohydrate drinks even if patient reports hunger; monitor for hypoglycemia in insulin‑treated patients kept NPO; ensure patient understands the rationale; do not withhold all fluids unnecessarily; balance patient preference with safety.
## Concrete Clinical Example
A 58‑year‑old woman with type 2 diabetes scheduled for elective colon resection had a preoperative order for a carbohydrate‑rich clear drink; the clinician changed the order to water only and explained the need to prevent preoperative hyperglycemia.
