# Lipoprotein(a) level ≥50 mg/dL as cardiovascular risk-enhancing factor
## STEP 1 — Gather Information
Collect personal/family history of premature ASCVD, personal ASCVD, or family history of high Lp(a); obtain Lp(a) level (mg/dL or nmol/L) via immunoassay (fasting or nonfasting acceptable).
## STEP 2 — Rule In / Rule Out
If Lp(a) ≥50 mg/dL (≥125 nmol/L), rule in as elevated Lp(a); otherwise rule out.
## STEP 3 — Classify or Stratify
If Lp(a) elevated, classify as a risk-enhancing factor; integrate with overall 10-year ASCVD risk (Pooled Cohort Equations) and other risk-enhancing factors.
## STEP 4 — Decide
For borderline/intermediate risk (5–19.9% 10-year ASCVD) with elevated Lp(a), consider CAC scoring to guide statin initiation; if high risk (>20%), intensify LDL-C–lowering therapy regardless of CAC.
## Clinical Guardrails / Mimics / Pitfalls
Lp(a) testing need not be repeated if previously measured; lowering Lp(a) not proven to reduce ASCVD risk; do not base statin decisions solely on Lp(a); Lp(a) unaffected by fasting status.
## Concrete Clinical Example
55‑year‑old man with father’s MI at 58, LDL‑C 130 mg/dL, 10‑year ASCVD risk 12%, Lp(a) 80 mg/dL → elevated Lp(a) → intermediate risk + risk‑enhancing factor → CAC score obtained; CAC=120 → initiate moderate‑intensity statin.
