# Consider DPP4 inhibitor with correction insulin or scheduled insulin for select mild hyperglycemia type 2 diabetes patients
## STEP 1 — Gather Information
Collect HbA1c (within past 3 months), current blood glucose, diabetes type, prior insulin use and total daily dose, hospitalization context (noncritical illness), and assess for contraindications (T1D, renal impairment, pancreatitis). If HbA1c <7.5%, BG <180 mg/dL, and (if insulin-naïve or TDD <0.6 units/kg) proceed; otherwise, consider alternative regimens.
## STEP 2 — Rule In / Rule Out
Rule in if: HbA1c <7.5% (9.4 mmol/L), BG <180 mg/dL (10 mmol/L), and (if previously on insulin) total daily dose <0.6 units/kg/day; rule out if any criterion fails or if T1D/other insulin-dependent diabetes is present.
## STEP 3 — Classify or Stratify
Stratify by patient preference and safety: prefer DPP4i + correction insulin for oral therapy desire and low hypoglycemia risk; choose scheduled insulin if DPP4i contraindicated (eGFR <30, history of pancreatitis), patient prefers insulin, or cost/access issues.
## STEP 4 — Decide
Initiate selected regimen: DPP4i (e.g., sitagliptin 100 mg daily) with correction insulin (rapid-acting analogue per sliding scale) OR scheduled insulin (basal ± nutritional insulin) targeting glucose 100–180 mg/dL.
## Clinical Guardrails / Mimics / Pitfalls
Do not use in type 1 diabetes, persistent hyperglycemia >180 mg/dL on DPP4i, eGFR <30 without dose adjustment, history of pancreatitis, or if patient cannot afford medication; avoid sliding scale insulin alone without scheduled basal in hyperglycemic patients.
