# Assess ability to operate CSII and make setting changes
## STEP 1 — Gather Information
Collect data on manual dexterity (button pressing, dial turning), visual acuity (reading display, distinguishing colors), mental health status (depression, anxiety), cognitive function (memory, executive function), and whether the patient operates the pump independently or with assistance. Document findings and note any specific deficits.
## STEP 2 — Rule In / Rule Out
Determine if the patient can independently operate the pump and make all necessary setting changes without assistance. If yes, rule in independent capability; if no, rule out independent capability and proceed to assess needed assistance level.
## STEP 3 — Classify or Stratify
For patients lacking independent capability, stratify the level of assistance required: (1) occasional assistance for complex tasks, (2) regular assistance for daily operations, (3) full dependence on caregiver for all pump functions. Assign an assistance level based on observed deficits.
## STEP 4 — Decide
If independent capability is confirmed, proceed with standard CSII education and training. If assistance is needed, tailor training to patient and caregiver, consider a pump with simplified interface or larger controls, and evaluate alternative insulin delivery methods if safety is a concern.
