# Against routine 25(OH)D testing in adults over 75
## STEP 1 — Gather Information
Collect patient age ≥75 years, confirm absence of symptoms suggestive of vitamin D deficiency (e.g., bone pain, proximal weakness), and assess for established indications for 25(OH)D testing such as hypocalcemia, hyperparathyroidism, osteoporosis treatment, malabsorption, or chronic kidney disease.
## STEP 2 — Rule In / Rule Out
Is there an established indication for 25(OH)D testing? If yes → consider testing per clinical context; if no → proceed to recommend against routine testing.
## STEP 3 — Classify or Stratify
Classify as asymptomatic older adult ≥75 years without established indications for 25(OH)D testing → no indication for routine 25(OH)D measurement.
## STEP 4 — Decide
Do not order a routine 25(OH)D test; if vitamin D supplementation is desired for mortality benefit, initiate empiric low-dose vitamin D (e.g., 800–1000 IU daily) without prior testing.
## Clinical Guardrails / Mimics / Pitfalls
Do not use 25(OH)D results to guide supplementation decisions in this population; avoid testing solely based on age or nonspecific symptoms like fatigue; do not repeat tests to monitor empiric supplementation; recognize that optimal 25(OH)D thresholds for benefit are unknown; note that empiric supplementation is inexpensive and feasible.
## Concrete Clinical Example
An 80‑year‑old man with no symptoms, no hypocalcemia, osteoporosis, or malabsorption asks whether his vitamin D level should be checked. Advise against routine testing and suggest starting empiric vitamin D 800 IU daily if not already supplemented.
