# Recommend gonadal hormone treatment for premenopausal women with central hypogonadism
## STEP 1 — Gather Information
Collect menstrual history (oligomenorrhea/amenorrhea), serum estradiol, FSH, LH, prolactin, TSH, free T4, and pregnancy test; exclude other causes (hyperprolactinemia, hyperandrogenism, thyroid disease); assess for contraindications (e.g., hormone-sensitive malignancy, active thromboembolism, unexplained vaginal bleeding, severe liver disease).
## STEP 2 — Rule In / Rule Out
Rule in central hypogonadism if estradiol is low with inappropriately low or normal FSH/LH and other causes excluded; otherwise rule out.
## STEP 3 — Classify or Stratify
If central hypogonadism is confirmed and no contraindications are present, classify as candidate for gonadal hormone therapy.
## STEP 4 — Decide
Prescribe gonadal hormone treatment (e.g., transdermal estradiol with cyclic progestogen for women with intact uterus; estrogen alone if hysterectomy) and arrange follow‑up for efficacy and adverse effects.

A 32‑year‑old woman with 6‑month history of amenorrhea, serum estradiol 20 pg/mL, FSH 3 mIU/mL, LH 4 mIU/mL, normal prolactin and thyroid, negative pregnancy test, and no contraindications starts transdermal estradiol 50 µg daily with cyclic oral progesterone 200 mg for 12 days/month.
