# Attempt desmopressin discontinuation post-pituitary surgery
## STEP 1 — Gather Information
Confirm patient is at least 1 week post-pituitary surgery (to avoid early postoperative risks), has DI requiring DDAVP, and is clinically stable. Collect baseline urine output, serum sodium, and ensure patient education on polyuria symptoms.
## STEP 2 — Rule In / Rule Out
Rule in: Patient is ≥1 week post-op, stable sodium (135-145 mmol/L), and no contraindications (e.g., adipsic DI without close monitoring). Rule out: <1 week post-op, unstable sodium, or inability to monitor urine output/symptoms.
## STEP 3 — Classify or Stratify
Discontinue DDAVP and monitor for 24-48 hours. Classify as recovered if urine output <3L/day, serum sodium stable, and no DI symptoms; classify as not recovered if polyuria (>3L/day), rising serum sodium, or symptoms of dehydration occur.
## STEP 4 — Decide
If recovered: discontinue DDAVP permanently. If not recovered: restart DDAVP at previous dose and consider re-attempting in 4-8 weeks.
## Clinical Guardrails / Mimics / Pitfalls
Do not attempt in first postoperative week (risk of hyponatremia/SIADH). Monitor for hyponatremia during trial (especially if fluid intake unrestricted). Avoid in adipsic DI without strict sodium monitoring. Educate patient to recognize polyuria and seek care. Do not discontinue if patient has comorbidities increasing risk (e.g., heart failure).
## Concrete Clinical Example
A 50-year-old patient 3 weeks post-pituitary adenoma resection on DDAVP 2 mcg BID for DI attempts discontinuation: stops DDAVP, monitors urine output. After 48 hours, urine output 1.5L/day, serum sodium 138 mmol/L, no symptoms. Clinician concludes posterior function recovered and discontinues DDAVP permanently.
