# Titrate desmopressin and fluid intake for adipsic diabetes insipidus
## STEP 1 — Gather Information
Collect baseline weight, daily fluid intake, serum sodium, urine output, and assess thirst mechanism.
→ Proceed to Step 2 once data are obtained.
## STEP 2 — Rule In / Rule Out
Confirm adipsic DI (lack of thirst response) and rule out other causes of hyponatremia (e.g., SIADH, glucocorticoid excess).
→ If adipsic DI confirmed, proceed to Step 3; otherwise, reconsider diagnosis.
## STEP 3 — Classify or Stratify
Classify serum sodium trend: stable (135‑145 mmol/L), mild hyponatremia (130‑134), moderate/severe (<130), or hypernatremia (>145).
→ Proceed to Step 4 based on classification.
## STEP 4 — Decide
For mild hyponatremia or weight gain: reduce DDAVP dose by 25‑50% and/or restrict fluid intake; for hypernatremia or weight loss: increase DDAVP dose or fluid intake; aim for stable weight and serum sodium 135‑145 mmol/L; reassess daily.
→ Continue monitoring and adjust as needed.

Avoid fixed DDAVP dosing without daily weight and sodium checks; do not impose fluid restriction without sodium monitoring; ensure a weekly off‑DDAVP period to allow polyuria; watch for over‑replacement causing hyponatremia and under‑replacement causing hypernatremia; educate patients on signs of fluid imbalance.
## Concrete Clinical Example
A 45‑year‑old post‑pituitary surgery patient with adipsic DI on DDAVP 10 µg BID reports stable weight and serum Na 138 mmol/L; after 3 days of weight gain 2 kg and Na dropping to 132 mmol/L, DDAVP is reduced to 5 µg BID and fluid intake limited to 1500 mL/day; daily weight and Na improve to ‑1 kg and 135 mmol/L, then dose is maintained.
