Record date/time, what was observed, severity or measurement when known, duration, context, relevant intake/activity, and what changed afterward. Preserve the user's wording where precision matters.
Keep observations separate from interpretations. Do not infer diagnoses. Highlight patterns only as patterns worth discussing with a qualified clinician. Avoid storing unnecessary sensitive detail.
