## Checklist
- Group into Added / Changed / Fixed / Removed; drop empty groups.
- Lead each entry with the user-visible outcome, not the internal module.
- Call out breaking changes first, with the migration in the same bullet.
- Skip refactors and chores unless a user can observe them.
- Link the issue or PR for anyone who needs the detail.
- Write in past tense, one line per entry.
