Trigger when something worth documenting just happened — a root cause, architecture decision, incident, meeting, milestone, people context, reusable pattern — on "make a note", on a subagent spawned to capture session work, and on repo onboarding ("remember this project").

"What should I write next?" and "is the vault rotting?" — vault health and gap finding. Trigger on what to document next, where notes are thin, vault cleanup / audit / "groom my memory", a recall that came up empty or hit `DANGLING`, or a Session/Project note that looks structurally wrong.

Check the vault before you re-derive. Trigger before reconstructing a past decision, root cause, incident, architecture rationale, or "why is it built this way" — even when no memory question is asked — and on explicit lookups ("what do I know about X", "check my notes").
