## PROCESS
### Step 0 — Offer preset (one message, before questions)

Presets don't skip OPERATOR_TOKEN and Phase Plan — always generate those.
**If user chose 5 or no answer** → go to Step 1 (4 questions).

Generate `plan.md` in the same directory as `CLAUDE.md`.
Format is optimized for machine read/write: YAML frontmatter for metadata,
consistent phase headers for grep, standard checkboxes for task tracking.

Rules:
