# Monitor lipid profile to identify dyslipidemia in Cushing syndrome
## STEP 1 — Gather Information
Obtain a lipid panel (total cholesterol, LDL-C, HDL-C, triglycerides) at the time of Cushing syndrome diagnosis and periodically thereafter as determined by the treating physician. Use fasting or nonfasting samples; repeat fasting if triglycerides are elevated or genetic dyslipidemia suspected.
## STEP 2 — Rule In / Rule Out
Rule in dyslipidemia if any lipid parameter exceeds normal ranges: LDL-C >70 mg/dL, triglycerides >150 mg/dL, HDL-C <40 mg/dL in men or <50 mg/dL in women. Rule out dyslipidemia if all parameters are within normal limits.
## STEP 3 — Classify or Stratify
If dyslipidemia is present, assess atherosclerotic cardiovascular disease (ASCVD) risk using the Pooled Cohort Equations and consider risk-enhancing factors (e.g., visceral obesity, hypertension, diabetes, elevated lipoprotein[a]).
## STEP 4 — Decide
If dyslipidemia is identified, advise lifestyle modification and consider statin therapy as adjunct to reduce cardiovascular risk, per guideline 7.2; if lipids remain normal, continue routine monitoring at physician discretion.
## Clinical Guardrails / Mimics / Pitfalls
Do not rely solely on total cholesterol; evaluate all lipid components. Avoid initiating statin therapy without confirming LDL-C >70 mg/dL, especially when patients are on ketoconazole (which increases statin levels and myopathy risk). Do not assume lipid-lowering therapy is appropriate for patients with limited life expectancy (e.g., underlying malignancy). Remember that triglycerides >500 mg/dL contraindicate bile acid sequestrants.
## Concrete Clinical Example
A 45-year-old woman with newly diagnosed adrenal adenoma–causing Cushing syndrome presents for initial evaluation. Lipid panel shows LDL-C 130 mg/dL, triglycerides 200 mg/dL, HDL-C 35 mg/dL. Dyslipidemia is ruled in; she is advised on diet and exercise, and statin therapy is considered after discussing risks/benefits.
