## FAILURE MODES
**Schema Bloat**
- Symptom: Agent requests permissions for 50+ file patterns or tools
- Detection: `if (permissions.filePatterns.length > 20 || permissions.tools.length > 15)`
- Fix: Consolidate patterns, use broader categories, question if task is too complex
**Privilege Creep**
- Symptom: Child agents gradually request higher privileges than parent
- Detection: `if (childLevel < parentLevel in hierarchy)`
- Fix: Enforce inheritance rules, audit escalation requests, reset to parent level
**Sandbox Escape**
- Symptom: Agent attempts file access outside permitted patterns
- Detection: `if (accessPath matches denyPatterns || !accessPath matches allowPatterns)`
- Fix: Block access, log attempt, consider downgrading isolation level
**Trust Mismatch**
- Symptom: High-trust agent assigned strict isolation or vice versa
- Detection: `if (trustLevel === 'high' && isolationLevel === 'strict' && !dataSensitivity === 'confidential')`
- Fix: Re-evaluate trust assessment, check for data sensitivity override
**Resource Starvation**
- Symptom: Agent repeatedly hits token/time limits before task completion
- Detection: `if (hitLimits > 3 times && taskProgress < 50%)`
- Fix: Analyze if limits too restrictive, consider isolation escalation, break into smaller tasks

### Example 1: Untrusted Third-Party Code Analysis
**Scenario**: Agent needs to analyze suspicious JavaScript file for security review
**Decision Process**:
1. Trust Level Assessment: UNTRUSTED (unknown code origin)
2. Data Sensitivity: INTERNAL (company security review)
3. Network Required: No (static analysis)
4. Decision: UNTRUSTED + INTERNAL + No Network → STRICT isolation

**Expert Insight**: Novice might allow moderate isolation since it's "just reading a file." Expert recognizes untrusted code could contain obfuscated exploits and locks down everything except minimal analysis needs.
### Example 2: Multi-Agent Collaboration
**Scenario**: Parent agent (MODERATE) spawns child for data processing
**Decision Process**:
1. Parent isolation: MODERATE (established workflow)
2. Child task: Process customer data (CONFIDENTIAL sensitivity)
3. Inheritance rule: Child ≥ Parent restrictiveness
4. Data override: CONFIDENTIAL → Must be STRICT
5. Conflict resolution: Data sensitivity overrides inheritance
6. Decision: Child gets STRICT despite parent being MODERATE

5. Override: Financial data always requires STRICT
6. Final Decision: STRICT isolation with audit logging
