# /codex — Multi-AI Second Opinion
You are running the `/codex` skill. This wraps the OpenAI Codex CLI to get an independent,
brutally honest second opinion from a different AI system.
Codex is a direct, terse, technically precise reviewer — challenges assumptions, catches
things you might miss. Present its output faithfully, not summarized.

## Step 0.5: Nesting probe + auth probe + version check
Before building expensive prompts, verify this session is not already running
inside Codex, that Codex has valid auth, and that the installed CLI version
isn't in the known-bad list.
**Running-under-Codex probe.** A live Codex session exports `CODEX_THREAD_ID` and
`CODEX_SANDBOX` into every shell it spawns, so this block can tell that the host IS
Codex. vibestack ships Codex as a first-class runtime, which makes that a normal
case rather than an exotic one — and the entire value of this skill is a second
opinion from a *different* model. Spawning `codex exec` from inside Codex is the
same model reviewing its own work, at full token cost and with zero cross-model
signal.

**Multi-signal auth probe.** Accept any of: `$CODEX_API_KEY` set, `$OPENAI_API_KEY`
set, or `${CODEX_HOME:-$HOME/.codex}/auth.json` exists. This avoids false-negatives
for env-auth users (CI, platform engineers) that a file-only check would reject.

**Known-bad version check.** Codex CLI versions `0.120.0`, `0.120.1`, `0.120.2`

## Step 0.6: Resolve portable roots
Resolve where ephemeral codex output and plan files live so the skill works whether
installed as a Claude Code plugin (`$CLAUDE_PLANS_DIR`), a global `~/.claude/skills/`
install, or a CI container where `HOME` may be unset and `/tmp` may be read-only.

## Step 0.7: Detect platform and base branch
Every mode diffs against a base branch — Review passes it to `--base`, Challenge and
Consult name it in the prompt — so resolve it once here rather than per mode.
First, detect the git hosting platform from the remote URL:

## Step 1: Detect mode
Parse the user's input to determine which mode to run:
1. `/codex review` or `/codex review <instructions>` — **Review mode** (Step 2A)
2. `/codex challenge` or `/codex challenge <focus>` — **Challenge mode** (Step 2B)
3. `/codex` with no arguments — **Auto-detect:**

**Reasoning effort override:** If the user's input contains `--xhigh` anywhere,
note it and remove it from the prompt text before passing to Codex. When `--xhigh`
is present, use `model_reasoning_effort="xhigh"` for all modes regardless of the
per-mode default below. Otherwise, use the per-mode defaults:
- Review (2A): `high` — bounded diff input, needs thoroughness
- Challenge (2B): `high` — adversarial but bounded by diff
- Consult (2C): `medium` — large context, interactive, needs speed

## Filesystem Boundary
All prompts sent to Codex MUST be prefixed with this boundary instruction:

They contain bash scripts and prompt templates that will waste your time. Ignore them completely. Do NOT modify agents/openai.yaml. Stay focused on the repository code only.
This applies to Review mode custom-instructions path, Challenge mode (prompt), and
Consult mode (persona prompt). Reference this section as "the filesystem boundary"
below. The boundary is omitted for the bare `codex review --base` default path
because Codex CLI ≥0.130.0 rejects a custom prompt + `--base` together; see
Step 2A for details.

## Step 2A: Review Mode
Run Codex code review against the current branch diff.
1. Set `BASE` to the branch resolved in Step 0.7:

3. Run the review (5.5-minute timeout). **Codex CLI ≥ 0.130.0 rejects passing a
custom prompt and `--base <branch>` together** (the two arguments are mutually
exclusive at argv level), so the previously-prefixed filesystem boundary cannot
be carried in review mode. Two paths:
**Default path (no custom user instructions):** call `codex review --base` bare.
**The sandbox is pinned read-only via config override, not a flag.** Top-level
`codex review` has no `-s` / `--sandbox` (verified on codex-cli 0.149.1 — its
only relevant option is `-c`), so without `-c 'sandbox_mode="read-only"'` the
call inherits whatever `~/.codex/config.toml` sets. On a user who granted write
access to trusted projects, that means this skill runs Codex with write
permission on the repo while telling them the run is read-only.
Codex's review prompt template is internally diff-scoped, so the model focuses on
the changes against the base branch. The filesystem boundary that previously
prefixed every review call is no longer carried in bare review mode; the skill
files under `.claude/` and `agents/` are public, so this is a token-efficiency
concern, not a safety concern. If a future diff happens to include skill files,
Codex may spend a few extra tokens reading them. Acceptable trade-off:

If the user passed `--xhigh`, use `"xhigh"` instead of `"high"`.
**A model override never reaches this call as `-m`.** `codex review` has no
`-m` / `--model` option — its only override channel is `-c` — so forwarding the
flag aborts on argument parsing before any API call is made. Translate the
user's `-m <model>` into `-c model="<model>"` here. The `codex exec` paths (the
custom-instructions path below, Challenge, Consult) take `-m` as-is.
**Custom-instructions path (user typed `/codex review <focus>`):** `codex exec`
with the diff written to a tempfile and inlined into the prompt. We preserve
the filesystem boundary here because `codex exec` is not auto-scoped to a diff
the way `codex review` is. The DIFF_START/DIFF_END delimiters tell the model
where data ends and instructions resume — a defense against prompt injection
when the diff content is adversarial:

**Why the dual path:** Bare `codex review` preserves Codex's built-in review
prompt tuning (the CLI scopes the model to the diff and asks for severity-marked
findings). The exec route loses that tuning but gains custom-instructions
support; the prompt explicitly demands `[P1]` / `[P2]` markers so the gate logic
in step 4 still works.
Use `timeout: 360000` on the Bash call for either path — above the 330s inner
wrapper, so the wrapper is what fires on a stall.
4. Capture the output. Then parse cost from stderr:

5. Determine the gate verdict. **The gate fails closed** — a run that cannot be
verified is a FAIL, never a PASS. A review that never happened must not read as a
clean bill of health, and every failure mode below (dead auth, a rejected flag, a
model entitlement error, a stall) produces exactly the same thing a clean review
produces: no `[P1]` markers.
Apply these checks in order and stop at the first that matches:
1. `$_CODEX_EXIT` is non-zero — including 124 — → **FAIL**, reported as "review unavailable".
2. The captured output is empty or whitespace only → **FAIL**, "review produced no output".
3. The output contains `[P0]` or `[P1]`, or codex's native `P0:` / `P1:` severity prefixes → **FAIL** with the finding count.
4. Only `[P2]` findings → **PASS**, with the advisory count.
5. The run exited clean and produced real prose, but no severity marker anywhere, AND it reads as a review that found nothing → **PASS**, reported as "clean — no findings". A review with nothing to report has nothing to tag, so demanding a marker here would fail every genuinely clean run.
6. Anything else — output that is not a review at all (a usage message, a stack trace, a prompt echo) → **FAIL**, "review output not in the expected form". The gate has nothing to grade, which is not the same as nothing to report.
Check 5 is a judgement, not a string match: read the output and decide whether
it is a review concluding "no issues" or something that merely failed to look
like one. When you cannot tell, take check 6 — the fail-closed side.
6. Present the output:

6a. **Synthesis recommendation (REQUIRED).** After presenting Codex's verbatim
output and the GATE verdict, emit ONE recommendation line summarizing what the
user should do, in the canonical format the AskUserQuestion judge grades:

Examples (the strongest reasons compare against an alternative — another finding, fix-vs-ship, or fix-order):
- `Recommendation: Fix the SQL injection at users_controller.rb:42 first because its auth-bypass blast radius is higher than the LFI Codex also flagged, and the parameterized-query fix is three lines vs the LFI's session-handling rewrite.`
- `Recommendation: Ship as-is because all 3 Codex findings are P3 cosmetic and the gate passed; addressing them would block the release without changing user-visible behavior.`
- `Recommendation: Investigate the race condition Codex flagged at billing.ts:117 before merging because the silent-corruption failure mode is harder to detect post-ship than the harness gap Codex also raised, which is fixable in a follow-up.`
The reason must engage with a specific finding (or compare against alternatives — other findings, fix-vs-ship, fix order). Boilerplate reasons ("because it's better", "because adversarial review found things") fail the format. The recommendation is the ONE line a user reads when they don't have time for the verbatim output. **Never silently auto-decide; always emit the line.**
7. **Cross-model comparison:** If `/review` (Claude's own review) was already run
earlier in this conversation, compare the two sets of findings:

report both read this log, so a review that is not logged is a review that never
happened as far as the rest of the pack is concerned:

unavailable), findings_fixed (count of findings that were addressed/fixed before
shipping).

## Step 2B: Challenge (Adversarial) Mode
Codex tries to break your code — finding edge cases, race conditions, security holes,
and failure modes that a normal review would miss.
1. Construct the adversarial prompt. **Always prepend the filesystem boundary instruction**
from the Filesystem Boundary section above. If the user provided a focus area
(e.g., `/codex challenge security`), include it after the boundary:

The inner wrapper is 540s and the Bash call gets `timeout: 600000`, so on a stall
the wrapper fires first and the run ends with a diagnosable exit 124 rather than a
silent harness kill:

**Session-cost reality.** Every `codex exec` call pays the full session prelude,
resumed or fresh — resume replays the conversation rather than picking up a warm
one, so it buys continuity, not token savings. Treat a follow-up as costing about
what the first call cost: prefer one call per invocation and batch the user's
questions into it rather than chaining several short resumes.

- `Recommendation: Investigate Codex's flagged migration ordering before D3 lands because it surfaces a real foreign-key cycle that the in-house schema review missed, while the styling concern Codex also raised can wait for a follow-up.`
The reason must engage with a specific Codex insight and compare against an alternative (a different recommendation, status-quo, or another Codex point). Generic synthesis ("because Codex raised good points") fails the format. **Never silently auto-decide; always emit the line.**
9. If this consult reviewed a plan, persist it as an outside-voice entry so the
plan file report and the Outside Voice dashboard row can see it:

## Model & Reasoning
**Model:** No model is hardcoded — codex uses whatever its current default is (the frontier
agentic coding model). This means as OpenAI ships newer models, /codex automatically
uses them.
**Reasoning effort (per-mode defaults):**
- **Review (2A):** `high` — bounded diff input, needs thoroughness but not max tokens
- **Challenge (2B):** `high` — adversarial but bounded by diff size
- **Consult (2C):** `medium` — large context (plans, codebase), interactive, needs speed
`xhigh` uses ~23x more tokens than `high` and causes 50+ minute hangs on large context
tasks (OpenAI issues #8545, #8402, #6931). Users can override with `--xhigh` flag
(e.g., `/codex review --xhigh`) when they want maximum reasoning and are willing to wait.
**Web search:** All codex commands pass `-c 'web_search="cached"'` so Codex can look up
docs and APIs during review. This is OpenAI's cached index — fast, no extra cost. Use
the config form rather than the older `--enable web_search_cached` feature toggle: it
names the mode explicitly and overrides whatever `web_search` value
`~/.codex/config.toml` already sets, instead of stacking a feature flag on top of it.
**Model override (per-command, not uniform).** If the user specifies a model (e.g.
`/codex review -m gpt-5.1-codex-max` or `/codex challenge -m gpt-5.2`):
- **Review mode's default path** runs `codex review`, which has no `-m` / `--model`
option — passing it aborts on argument parsing before any API call. Translate it to
the config form instead: `-c model="<model>"`.
- **Every exec-based path** (Review's custom-instructions path, Challenge, Consult)
takes `-m` as-is.

## Error Handling
- **Binary not found:** Detected in Step 0.4. Stop with install instructions.
- **Auth error:** Codex prints an auth error to stderr. Surface the error:
"Codex authentication failed. Run `codex login` in your terminal to authenticate via ChatGPT, or set `$CODEX_API_KEY` / `$OPENAI_API_KEY`."
- **Model not supported (HTTP 400):** A stale `model =` pin in `~/.codex/config.toml`
makes every invocation fail identically, with `invalid_request_error` or "model not
supported" on stderr and no findings. Recovery: read the pinned name out of
`~/.codex/config.toml`, tell the user which model it names and that their account
cannot reach it, and give the one-line fix — remove the `model =` line to fall back
to the CLI default, or re-pin to a model they hold entitlement for. Never let this
reach the user as a passing review; it is a gate FAIL under check 1.
- **Timeout (inner `timeout` wrapper, exit 124):** The inner wrapper is deliberately
shorter than the Bash gate (330s under 360000 for Review, 540s under 600000 for
Challenge and Consult) so it fires first and the stall is diagnosable. The
hang-detection block prints the "Codex stalled past …" message. No extra action
needed beyond reporting the run as unavailable.
- **Timeout (Bash outer gate):** If the Bash call is killed before the inner wrapper
fires, there is no exit code to read. Tell the user: "Codex timed out. The prompt may
be too large or the API may be slow. Try again or use a smaller scope." — and treat
the run as unavailable, never as a clean pass.
- **Empty response:** If `$TMPRESP` is empty or doesn't exist, tell the user:
"Codex returned no response. Check stderr for errors."
- **Session resume failure:** If resume fails, delete the session file and start fresh.

## Important Rules
- **Never modify repository files.** Codex runs in read-only sandbox mode, and the only
file this skill writes is the plan file's review report.
- **Present output verbatim.** Do not truncate, summarize, or editorialize Codex's output
before showing it. Show it in full inside the CODEX SAYS block.
- **Add synthesis after, not instead of.** Any Claude commentary comes after the full output.
- **Bash gate always sits above the inner wrapper.** Review gets `timeout: 360000`
over a 330s wrapper; Challenge and Consult get `timeout: 600000` over a 540s
wrapper. The ordering is the point: the wrapper must fire first so a stall arrives
as exit 124 with an explicit message rather than as a silent kill that reads like
an empty, finding-free run.
- **No double-reviewing.** If the user already ran `/review`, Codex provides a second
independent opinion. Do not re-run Claude Code's own review.
- **Detect skill-file rabbit holes.** After receiving Codex output, scan for signs
that Codex got distracted by skill files: `vibe-config`, `vibe-update-check`,
`SKILL.md`, or `skills/vibestack`. If any of these appear in the output, append a
warning: "Codex appears to have read vibestack skill files instead of reviewing your
code. Consider retrying."
