## Common Pitfalls
**1. Policy bloat without enforcement.** Drafting dozens of policies that nobody reads or enforces creates a false sense of security. Every policy must have an owner, a review cadence, and a measurable enforcement mechanism — from automated RBAC checks to quarterly attestation campaigns.
**2. Role explosion in RBAC.** Granting custom roles for every edge case leads to an unmanageable matrix of permissions that nobody can audit. Enforce a tiered RBAC model (User → Power User → Admin → Super Admin) and resist requests for one-off roles. Use ABAC for fine-grained decisions where RBAC tiers are too coarse.
**3. Compliance checkbox mentality.** Treating SOC2 or ISO 27001 as a one-time audit event rather than a continuous process. Certifications require ongoing evidence collection, periodic risk assessments, and remediation tracking — not just a binder of policies filed after the auditor leaves.
**4. Audit trail gaps.** Logging who accessed what is half the story. Without tamper-proof storage, correlation IDs, and regular log review, an audit trail becomes a liability. Ensure logs are append-only (e.g., journald, cloud trail, blockchain-anchored hashes), retained per regulatory requirements, and reviewed on a schedule.
**5. Transparency without governance.** Publishing everything openly without access controls or approval workflows invites chaos. Radical transparency should apply to outcomes and decisions, not to raw credentials, unreviewed policy drafts, or PII. Layer governance processes (review gates, approval chains, escalation paths) on top of transparent reporting.
**6. Ignoring organizational inertia.** The best governance framework fails if the team is not trained, motivated, or held accountable. Pair every new policy with training, a grace period, and a feedback loop that allows the policy to adapt to actual workflows.
