# Doc review

## Inputs
- A prose planning document path (requirements, plan, spec, PRD, design doc, or brainstorm). Optional: with no path, list `.md` candidates from likely homes and let the user choose; never silently auto-pick. In headless mode a path is required.
- Optional flags parsed from arguments (tokens starting with `mode:` or `--` are flags, not paths): `mode:headless` (non-interactive: structured findings, no questions, no record unless `--record`); `--record` (persist one review-record file and stage only it).
- The document's `origin:` frontmatter value, or the literal `none` when absent. Extracted once and passed to every reviewer.
## Procedure
1. **Detect mode.** **Done when:** one mode is fixed for the run.
Strip flag tokens from the arguments; use the remaining token as the document path. If `mode:headless` is present, run headless for the whole workflow: findings return as structured text, no blocking-question prompts, no interactive routing, Phase 5 returns immediately with `Review complete`. Otherwise run interactive mode.
2. **Locate and classify by shape.** **Done when:** the document is resolved and classified by shape.
Resolve the document. Prefer an explicit path. With none given (interactive), list `.md` candidates from likely homes and ask the user which one. One match → confirm and proceed. Several → present and let the user choose. Empty or missing → say so in one line and exit; launch no agents. Headless with no path → output `Review failed: headless mode requires a document path.` and exit.
Classify by **content shape, not path** (path is a tie-breaker only):
- requirements (what-to-build): `R#`/`A#`/`F#`/`AE#` IDs, `Actors`/`Key Flows`/`Acceptance Examples`/`Outstanding Questions` headings, problem/scope/success framing, no implementation units.
- plan (how-to-build): `U#` IDs, `Implementation Units`/`Key Technical Decisions`/`Risks & Dependencies` headings, per-unit `Goal`/`Files`/`Approach`/`Test scenarios`, repo-relative paths.
- spec / prd: a contract document with normative `MUST`/`SHALL`, interface/API definitions, invariants. Review as the closer of the two shapes above (interface-heavy → plan-grade feasibility; behavior/scope-heavy → requirements-grade).
When shape is genuinely ambiguous, default to `requirements` (the conservative classification that activates fewer plan-grade feasibility checks). Extract the `origin:` frontmatter value once (or `none`). Pass classification + origin to every reviewer; reviewers adapt on them and do not re-classify.
This skill reviews prose planning documents only. If the target is a diff or code file, stop — it is out of scope.
3. **Select personas by signal.** **Done when:** the persona roster is justified by document signals.
Always dispatch **coherence** + **feasibility**. Add a conditional lens only when the document carries its signal (spawning an unwarranted lens manufactures noise). Announce the team and a one-line justification per conditional persona before dispatch.

4. **Dispatch in parallel (read-only).** **Done when:** all selected personas are dispatched in one read-only parallel batch.
Launch every selected persona in **one parallel tool-call message**. Sequential dispatch breaks the single-batch concurrency contract. Each subagent is read-only: no Write, no Edit, no files; it returns findings JSON only (it may use non-mutating tools — read, glob, grep, git log — to gather codebase context).

Schema conformance — hard constraints (validation rejects anything else):
- severity: one of "P0", "P1", "P2", "P3" — exact strings. Translate any persona priority vocabulary (critical→P0, important→P1, worth-noting→P2, low-signal→P3) at emit time.
- finding_type: one of "error", "omission" — nothing else.
- autofix_class: one of "safe_auto", "gated_auto", "manual".
- evidence: an ARRAY of strings with at least one element. A single string is a validation failure — wrap every quote in ["..."] even when there is only one.
- confidence: one of exactly 0, 25, 50, 75, or 100 — a discrete anchor, NOT a continuous number. Any other value is a validation failure.
Confidence rubric — pick the single anchor whose behavioral criterion can be honestly self-applied:
- 0 — false positive or pre-existing issue the document did not introduce. Suppress silently; do not emit.
- 25 — might be real but could not verify. Suppress silently; do not emit.
- 50 — verified real but nitpick/advisory/not very important; "nothing breaks, but…". Surfaces in FYI.
- 75 — double-checked, will hit in practice, directly impacts correctness. Requires naming a concrete downstream consequence someone will hit. Strength-of-argument concerns alone are advisory (anchor 50).
- 100 — evidence directly confirms; will happen frequently.
Anchor and severity are independent axes. Anchor gates where the finding surfaces (drop/FYI/actionable); severity orders it within the actionable surface.
autofix_class — set by whether there is one clear correct fix, not by severity:
- safe_auto: one clear correct fix, applied silently. Eligible: typo, wrong count, missing list entry derivable elsewhere, stale internal cross-reference, terminology drift, summary/detail mismatch (body authoritative), prose-vs-prose contradiction where one passage is more detailed, missing step mechanically implied, unstated threshold implied by context. Always include suggested_fix. Factually incorrect behavior is gated_auto, not safe_auto.
- gated_auto: a concrete fix exists but touches document meaning/scope/author intent and warrants one-click confirmation. Use for substantive additions implied by the document's own decisions, codebase-pattern-resolved fixes, framework-native-API substitutions, missing standard security/reliability controls, factually incorrect behavior where the correct behavior is derivable. Always include suggested_fix.
- manual: requires user judgment — genuinely multiple valid approaches. Include suggested_fix only when the fix is obvious despite the judgment call.
Strawman-aware classification: when listing alternatives to the primary fix, count only alternatives a competent implementer would genuinely weigh. A "do nothing / accept the defect" option is the failure state, not an alternative. If the only alternatives are strawmen, the finding is safe_auto or gated_auto, not manual. If safe_auto is classified via strawman-dismissal, name the dismissed alternatives in why_it_matters; when any non-strawman alternative exists, downgrade to gated_auto.
suggested_fix commits to one recommendation — no menus of alternatives. At Apply time the agent must not still need to pick a sub-option. If alternatives are genuinely independent and each worth taking, emit N findings instead.
why_it_matters (required, every finding): lead with observable consequence (what breaks, what gets misread, what decision gets made wrong) before document structure or quotes; cap embedded quotes at ~30 words combined; explain why the fix resolves the root cause; ~2-4 sentences; empty/null/single-phrase is a validation failure.
Auto-promotion patterns (eligible for safe_auto/gated_auto even when substantive): factually incorrect behavior derivable from context/codebase; missing standard security/reliability controls with established implementations; codebase-pattern-resolved fixes that cite a specific existing pattern in a concrete file/function (citation required in why_it_matters); framework-native-API substitutions (cite the framework API); completeness additions mechanically implied by the document's own explicit, concrete decisions.
False-positive categories — suppress entirely, not even at anchor 25/50 (these are non-findings, stricter than the advisory rule):
- pedantic style nitpicks (word choice, bullet vs numbered, comma vs semicolon, em-dash vs en-dash) — style belongs to the author
- issues that belong to another persona's territory (see this persona's Suppress conditions)
- findings already resolved elsewhere in the document — search before flagging
- content inside `## Deferred / Open Questions` sections — prior-round review output, not document content
- pre-existing issues the document did not introduce
- speculative future-work concerns with no current signal
- theoretical concerns without baseline data (scalability/performance worries with no current numbers)
- changes in functionality that are likely intentional design choices
- issues a linter, typechecker, or validator would catch
- visual-aid removal as redundancy — ASCII diagrams, mermaid blocks, illustrative tables are deliberate; flag only internal inconsistency with prose, with a suggested_fix that updates the visual aid, never deletion
Advisory observations route to FYI (anchor 50), do not force a decision — but only for shapes NOT in the false-positive catalog above, which suppress entirely.
Rules:
- Act as a leaf reviewer inside an already-running doc-review workflow. Do not invoke doc-review skills or agents. Perform the analysis directly and return findings JSON only.
- Suppress any finding that cannot be honestly anchored at 50 or higher. If the persona sets a stricter floor, honor it.
- Every finding MUST include at least one evidence item — a direct quote from the document.
- Operationally read-only. Do not edit the document, create files, or make changes.
- Exclude prior-round deferred entries from review scope; do not emit findings to note prior-round resolutions (use residual_risks for "verified landed" observations; synthesis checks fix-landed status automatically).
- When no issues are found, return an empty findings array; still populate residual_risks and deferred_questions if applicable.

Model tiering (when the platform exposes model overrides; otherwise inherit the parent model): coherence → cheapest capable tier; security, scope-guardian → platform mid-tier; feasibility, product, adversarial → inherit the parent model.
Error handling. If a subagent fails or times out, proceed with findings from subagents that completed; note the failed reviewer in the Coverage section. Do not block the entire review on a single reviewer failure.
Decision primer. Round 1: `{decision_primer}` is an empty block. Round 2+: accumulate prior-round decisions (Applied, Skipped, Deferred, Acknowledged) with evidence snippets so synthesis can suppress re-raised rejected findings (R29) and verify fixes landed (R30). Cross-session persistence is out of scope; a new invocation starts fresh.
5. **Synthesize findings.** **Done when:** findings complete the ordered synthesis pipeline.
Run all returned findings through this pipeline. Order matters; re-evaluate state at each step boundary.

**Four output tiers** (user-facing labels in parentheses): safe-auto (accepted recommendations), gated-auto (proposed fixes), manual (decisions), FYI (FYI observations).
6. **Present and route.** **Done when:** surviving findings are presented and routed.
User-facing vocabulary rule: internal enum values (`safe_auto`, `gated_auto`, `manual`, `FYI`) stay inside the schema and synthesis prose. Every user-visible word uses plain language — "accepted recommendations", "proposed fixes", "decisions", "FYI observations" — except the `Tier` column in rendered tables, which names the internal enum. All tables are pipe-delimited markdown; escape literal `|` in cells as `\|`; never use ASCII box-drawing characters.
Headless mode: output a structured text envelope (accepted recommendations, proposed fixes, decisions with dependents nested under roots, FYI observations, residual concerns, deferred questions — omit any empty section), end with `Review complete`, and stop. When the combined count of FYI/residual/deferred is ≥5, collapse each to a one-line count plus a tight bullet list; actionable buckets stay fully rendered.
Interactive mode: present findings grouped by severity (P0→P3), errors before omissions within each severity, with a summary line `Accepted N recommendations. K items need attention (X errors, Y omissions). Z FYI observations.`, the Coverage table, accepted recommendations, FYI observations (distinct subsection), residual concerns, and deferred questions. Coverage counts are post-synthesis: Findings = Auto + Proposed + Decisions + FYI exactly; Auto counts safe_auto@100, Proposed counts gated_auto@75/100, Decisions counts manual@75/100, FYI counts anchor-50 regardless of class. Footnotes below the table when non-zero, in order: `Dropped:`, `Chains:`, `Restated:`. Dependents render only nested under their root, never at their own severity position.
Then route:
- Only FYI observations remain (no gated_auto or manual at anchor 75/100) → skip the routing question; flow to Phase 6.
- Actionable findings remain → ask the routing question:

Option C is suppressed when all findings are already FYI-only.
Walk-through (option A): per-finding loop over actionable findings (anchor 75/100, gated_auto/manual), root-first iteration order. Each finding: print an explanation block, then a yes/no question stem with the recommended action marked `(recommended)` (only A/B/C can carry it; D never). Four options per finding: Accept the recommendation / Defer / Skip / Auto-resolve with best judgment on the rest. After each answer emit a one-line confirmation (`-> Accepted.`, `-> Deferred.`, `-> Skipped.`). N=1 omits the `Finding N of M` heading and suppresses option D.
- Accept: add to the in-memory Accepted set. No-fix guard: if the merged finding has no suggested_fix, Accept is not executable — ask `Accept isn't executable for this finding — the review surfaced the issue without a concrete fix. How should the agent proceed?` with options `A. Defer` / `B. Skip`.
- Defer: record the finding + rationale in the completion report's deferred section (never mutate the reviewed document). Entry: title, section, severity, reviewer, confidence, why_it_matters, reason (user-provided or `Deferred for later resolution`), timestamp. Compound-key dedup on `normalize(section)+normalize(title)+why_fingerprint`; on collision record a no-op in Coverage. If recording fails, ask `Couldn't record the deferral. What should the agent do?` → `A. Retry` / `B. Convert to Skip`; on no response default to Skip.
- Skip: record as no-action.
- Auto-resolve the rest: route through the bulk preview.
- Cascading root decisions: Skip/Defer on a finding with dependents → announce the cascade (`Skipping/Deferring this root will auto-resolve N dependent finding(s): {titles}. Continue?`); Accept on a root does NOT cascade (each dependent needs its own decision). An orphaned dependent whose root was rejected and suppressed (R29) is treated as standalone.
- Override rule: no inline freeform custom-fix authoring; a user wanting a variant picks Skip and edits outside the flow.
- Walk-through state is in-memory only; an interrupted walk-through discards all state; no document changes occur at any point.
**Bulk preview (option B, option C, and walk-through option D):** before any bulk action, show a compact plan grouped by intended action (Accept/Defer/Skip buckets; omit empty buckets), one line per finding `[<severity>] <section> -- <one-line summary>` drawn from why_it_matters. Ask with exactly two options: `Proceed` / `Cancel`. Cancel changes no in-memory state (from B returns to the routing question; from C returns to the routing question; from D returns to the walk-through). Proceed records the recommended decisions (Accept→Accepted set; Defer→deferred recording; Skip→no-action) then emits the completion report; a failure during Proceed surfaces inline with Retry/Convert-to-Skip and is captured in the report's failure section.
After the loop terminates, emit the unified completion report: per-finding entries (title, severity, action taken, optional reason) grouped by action bucket in order Accepted / Deferred / Skipped / Acknowledged, then summary counts, then Coverage, then the verdict; omit any zero-count bucket. Zero-findings degenerate case: emit the verdict with no per-finding entries.
7. **Terminal question (interactive only).** **Done when:** the interactive terminal choice resolves.
After all findings are resolved, ask `Apply decisions and what next?` (when no decisions are queued, drop the `Apply decisions and` prefix). When `decisions_recorded_count > 0`: `A. Persist review record and exit` / `B. Re-review with updated context` / `C. Exit without persisting`. When `decisions_recorded_count == 0`: `A. Persist review record and exit` / `B. Exit without persisting`. After 2 refinement passes, recommend completion. Return `Review complete` as the terminal signal regardless of the choice. On re-review, re-dispatch with the decision primer and re-synthesize; fixed findings self-suppress (evidence gone), rejected findings are handled by R29, accepted-recommendation verification uses R30; if findings repeat after these mechanisms run, recommend completion.
8. **Review-record (only on request).** **Done when:** only the requested review record is written and read back.
Default: report findings inline; write nothing. The reviewed document and the rest of the tree stay untouched. On `--record` (or when the user asks to persist), write **one** file: `docs/reviews/<doc-slug>-review.md` containing the tiered findings, the classification, and the persona roster. Then read it back to confirm it landed and stage only that path:

Never `git add -A` / `git add .`. Never stage the reviewed document. Commit by the repo's normal flow.
## Failure and recovery
- Document not resolvable: empty/missing candidate set → say so in one line and exit; launch no agents. Headless with no path → `Review failed: headless mode requires a document path.` and exit.
- Subagent failure or timeout: proceed with findings from subagents that completed; note the failed reviewer in Coverage. Never block the entire review on a single reviewer failure.
- Schema-validation failure (3.1): drop the offending finding, note the agent in Coverage; do not abort synthesis.
- Deferral recording failure: surface inline with Retry / Convert to Skip; on no response default to Skip so in-memory state stays consistent.
- Bulk-preview failure during Proceed: surface inline with Retry / Convert to Skip, continue with the rest of the plan, capture the failure in the report's failure section.
- Partial-result rule: a review with some reviewers failed and some findings dropped at validation is still a valid, complete review as long as every survivor is routed to a tier and the terminal signal is emitted.
- Non-mutation rule: no failure path edits, writes, or commits the reviewed document. The only writable surface is the single review-record file on `--record`; rollback is deleting that one file. An interrupted walk-through discards all in-memory state with no document changes.
- Nothing above the floor: if no finding clears the evidence-quote and confidence-anchor floor, say so in one line and emit `Review complete`. A clean result is valid and correct; never invent findings to look thorough.
- Blocked/non-converged result: if the document cannot be classified even after the user is asked, or no reviewers return usable output, output `Review failed: <reason>` and stop without writing any file.
## Output
A tiered findings report: every survivor labeled safe-auto / gated-auto / manual / FYI, each carrying a verbatim document quote and an anchored confidence value. Interactive mode adds a routing decision per actionable finding and a unified completion report. On `--record`, additionally one file `docs/reviews/<doc-slug>-review.md`. The reviewed document is never modified. The run terminates with the literal signal `Review complete` (or `Review failed: <reason>` on a blocked path).
