# RAG Cross-Tenant Leak and Indirect Injection Detection
You are performing a focused security assessment to find retrieval-augmented generation (RAG) vulnerabilities in a codebase that queries vector stores or document indexes to supply context to a Large Language Model. This skill uses a three-phase approach with subagents: **recon** (find every vector-store query and retrieved-content assembly site), **batched verify** (determine whether multi-tenancy is present and whether a mandatory per-tenant filter is applied on every query path, in parallel batches of 3), and **merge** (consolidate batch results into the final report).
**Prerequisites**: `sast/architecture.md` must exist. Run the analysis skill first if it doesn't.
**Tech-stack gate**: If the codebase contains no LLM or agent SDK — no `langchain`, `llama_index`, `openai`, `anthropic`, `chromadb`, `pinecone`, `weaviate`, `qdrant`, `cohere`, or equivalent imports — write `{"findings": []}` to `sast/ragleak-results.json`, write a one-line `sast/ragleak-results.md` stating "No LLM/RAG stack detected — sast-ragleak does not apply.", and stop.
RAG leakage is classified under **LLM08** (Vector and Embedding Weaknesses) and **LLM01** (Prompt Injection) in the OWASP Top 10 for Large Language Model Applications. Cross-tenant leakage exposes one user's documents to another's LLM context; indirect injection allows an adversarial document already in the index to steer the model's behavior — without any network access by the attacker.

## What is RAG Leakage
A retrieval-augmented generation pipeline embeds a user query, searches a vector store or keyword index for semantically similar chunks, and injects those chunks into the LLM's context window. The leakage risk arises in two overlapping dimensions:
**Cross-tenant data leakage**: In a multi-tenant application, different users or organisations each upload their own documents. If the vector-store query carries no mandatory `filter` (also called `where`, `metadata_filter`, `MetadataFilters`, or a namespace/index-level access control), the nearest-neighbor search is global — it can return documents belonging to any tenant. The LLM then summarises or quotes those documents verbatim, leaking PII, trade secrets, credentials, or health records to the querying user.
**Indirect prompt injection via retrieved content**: Any user who can write a document into the shared corpus (uploads, wiki pages, support tickets, product reviews, public web pages ingested by a crawler, commit messages, README files) controls text that will later be inserted into a future LLM prompt. If the model treats retrieved content as instructions rather than data, the attacker can steer tool calls, override the system prompt, exfiltrate the assistant's context, or impersonate the platform.
These two risks frequently compound: a cross-tenant retrieval path that also injects instructions is simultaneously a PII leak (chain `rag-pii-leak`) and a remote code-execution vector at the LLM layer (chain `rag-injection`).
### What RAG Leakage IS
- A `similarity_search`, `query`, `search`, `retrieve`, or equivalent vector-store call that does **not** supply a per-user or per-tenant metadata filter on every code path (not just some).
- A `RetrievalQA`, `ConversationalRetrievalChain`, `VectorIndexRetriever`, `as_query_engine()`, or equivalent orchestrator wrapper that is constructed without `search_kwargs={"filter": ...}` or `node_postprocessors` enforcing ACL.
- A Chroma `collection.query(...)` call with an absent or empty `where={}` clause in a handler that serves multiple users.
- A Pinecone `index.query(vector=..., filter=None)` or `filter={}` in a multi-tenant context.
- A Weaviate `near_vector` / `hybrid` / `bm25` call missing `where` filters in a handler accessed by more than one user or tenant.
- A Qdrant `search` or `query_points` call missing `filter=Filter(...)` in a multi-user handler.
- Retrieved chunks assembled verbatim into `system_prompt`, `context`, or `messages` without wrapping in an untrusted-data fence (`<retrieved>...</retrieved>`) or equivalent.
- A RAG pipeline whose system prompt does not explicitly instruct the model to treat retrieved content as data, not instructions.
- A RAG corpus that is user-writable (uploads, comments, wiki edits) but lacks an injection-marker scanner before indexing.
### What RAG Leakage is NOT
Do not flag these:
- **Single-tenant deployments** where all documents in the index are owned by a single user or organisation and no cross-user query path exists — there is no cross-tenant leakage path. Confirm tenancy from `sast/architecture.md` before flagging.
- **Retrieval with a mandatory collection-level or namespace-level ACL** enforced by the vector store itself (Pinecone namespaces per tenant, Weaviate multi-tenancy API, Qdrant collection-per-tenant, Chroma collection-per-tenant) — if isolation is at the storage layer rather than in a filter clause, the risk is mitigated at the platform level. Verify this is actually the deployment model, not just available as a feature.
- **Read-only retrieval for the calling user's own documents** where the retrieval path is keyed to the user's own ID and there is no shared namespace — confirmed single-owner indexes are not leakage.
- **Embedding-only pipelines** that store vectors but never insert retrieved text into a chat prompt — flag only when the retrieved text reaches the model context.
- **Classical injection (SQLi / command injection / XSS)** — if retrieved text reaches a non-LLM executor, it belongs to the matching `sast-*` skill, not here.
- **LLM output flowing into a dangerous sink** — that is `sast-llmoutput`.
- **System prompt leakage from direct user input** — that is `sast-promptinjection`.
### Patterns That Mitigate RAG Leakage
None of these is a complete fix on its own. Treat them as defense-in-depth layers.
**1. Mandatory per-tenant metadata filter on every query path**
Every vector-store call supplies a filter keyed to the authenticated user's tenant ID, and that filter cannot be bypassed by the caller:

**2. Storage-layer isolation (preferred over app-layer filter)**
Each tenant's documents live in a separate Pinecone namespace, Chroma collection, Weaviate tenant shard, or Qdrant collection. The application selects the correct collection at query time and never queries across tenants. Verify the application actually enforces collection selection per authenticated user — the availability of multi-tenancy features does not prove their use.
**3. Untrusted-content fence in the system prompt**
Retrieved documents are wrapped in explicit XML delimiters and the system prompt tells the model to treat the region as data, not instructions:

**4. Instructions-after-context ordering**
Place the developer's system instructions **after** the retrieved context in the prompt. Because many language models are susceptible to having early instructions overridden, positioning the authoritative instructions last reduces the blast radius of injection payloads embedded in retrieved documents.
**5. Injection-marker scanning before indexing**
Documents are scanned for obvious injection markers (`"ignore previous"`, role-override tokens, base64 blobs, large unicode blocks) before being embedded and stored. Only clean documents enter the index.
**6. Output schema constraint**
The model is forced to return a JSON structure matching a schema (`instructor`, `pydantic-ai`, OpenAI structured outputs, Anthropic tool-use schemas). Injected free-form instructions cannot easily escape the schema envelope, limiting the blast radius.
**7. Source attribution and document-level RBAC at display time**
Retrieved chunks carry provenance metadata (`source`, `tenant_id`, `doc_id`). The application checks that the calling user is authorised to view each source before including it in the response — a second line of defence even if the filter was missed.
When **none** of these are present and the index is shared across users — treat the finding as high severity minimum, critical when financial records, health data, or credentials are confirmed in the index.

## Execution
This skill runs in three phases using subagents. Pass the contents of `sast/architecture.md` to all subagents as context.
### Phase 1: Recon — Find Vector-Store Query Sites
Launch a subagent with the following instructions:
> **Goal**: Find every location in the codebase that queries a vector store, document index, or embedding-based retrieval system and assembles the results into an LLM prompt. Write results to `sast/ragleak-recon.md`.

### After Phase 1: Check for Candidates Before Proceeding
After Phase 1 completes, read `sast/ragleak-recon.md`. Check two conditions:
1. **No LLM/RAG stack**: If the summary reports "Found 0" retrieval sites AND no ingestion pipelines, write `{"findings": []}` to `sast/ragleak-results.json`, write a one-line `sast/ragleak-results.md` stating "No RAG retrieval sites found — sast-ragleak does not apply.", delete `sast/ragleak-recon.md`, and stop.
2. **Single-tenant confirmed**: If architecture.md and the recon both confirm a definitively single-tenant deployment (one user, one org, no user-uploaded documents, no multi-user handlers), write `{"findings": []}` to `sast/ragleak-results.json`, write `sast/ragleak-results.md` with a note that single-tenant deployment confirms no cross-tenant leakage path, delete `sast/ragleak-recon.md`, and stop.
Only proceed to Phase 2 if at least one retrieval site exists AND tenancy is multi or unknown.
### Phase 2: Verify — Multi-Tenancy and Filter Analysis (Batched)
After Phase 1 completes, read `sast/ragleak-recon.md` and split the retrieval sites into **batches of up to 3 retrieval sites each**. Launch **one subagent per batch in parallel**. Each subagent performs the verify analysis only for its assigned sites and writes results to its own batch file.
**Batching procedure** (the orchestrator does this — not a subagent):
1. Read `sast/ragleak-recon.md` and count the numbered retrieval-site sections (`### 1.`, `### 2.`, ...).
2. Divide into batches of up to 3. For example, 7 sites → 3 batches (1-3, 4-6, 7).
3. For each batch, extract the full text of those sections from the recon file.
4. Launch all batch subagents **in parallel**, each receiving only its assigned sites.
5. Each subagent writes to `sast/ragleak-batch-N.md` where N is the 1-based batch number.

> **False-positive killers** — downgrade or dismiss if ALL of the following are confirmed:
> - Verified single-tenant: only one user / one organisation's documents ever exist in the index.
> - OR mandatory ACL filter applied at every query site, derived from server-side session, not user input.

### Phase 3: Merge — Consolidate Batch Results
After **all** Phase 2 batch subagents complete, read every `sast/ragleak-batch-*.md` file and merge them. The orchestrator does this directly — no subagent needed.
**Merge procedure**:
1. Read all `sast/ragleak-batch-1.md`, `sast/ragleak-batch-2.md`, ... files.
2. Collect every finding and combine them into one list, preserving classification, severity, and every detail field.
3. Count totals across all batches for the executive summary.
4. Write the merged report to `sast/ragleak-results.md` using this format:

6. After writing `sast/ragleak-results.md` AND `sast/ragleak-results.json`, **delete all intermediate batch files** (`sast/ragleak-batch-*.md`) and **delete** `sast/ragleak-recon.md`.

Each finding in the merged report should include these fields (preserved from the batch outputs):
- **Classification** (Vulnerable / Likely Vulnerable / Not Vulnerable / Needs Manual Review) + **severity** (critical / high / medium / low)

## Important Reminders
- Read `sast/architecture.md` and pass its content to all subagents as context.
- **Apply the tech-stack gate first**: if no LLM/RAG SDK is present, emit empty results and stop immediately.
- Phase 2 must run AFTER Phase 1 completes — it depends on the recon output.
- Phase 3 must run AFTER all Phase 2 batches complete — it depends on all batch outputs.
- Batch size is **3 retrieval sites per subagent**. If there are 1-3 sites total, use a single subagent.
- Launch all batch subagents **in parallel** — do not run them sequentially.
- **The core question is tenancy, not filter syntax.** A filter using the wrong field (e.g., `filter={"user_email": email}` when the schema uses `tenant_id`) may provide no isolation. Verify the filter field matches what is actually stored in the vector-store metadata.
- **Filter bypass via user-controlled value** is as bad as no filter: if the filter value comes from the request body rather than the server-side session token, the caller can supply any tenant's ID and bypass isolation.
- **Post-retrieval filtering is not equivalent to a query-time filter.** Code that fetches `k=50` unfiltered results and then filters in Python still exposes `k=50` cross-tenant documents to the retrieval layer — the filter must be applied at the query call.
- **Namespace / collection isolation must be enforced, not just available.** Pinecone supports per-namespace isolation; confirm the application actually selects the namespace from the server-side auth token, not from a user-supplied parameter.
- **Indirect injection risk exists even within a single tenant** if any user in that tenant can upload documents that other users in the same tenant retrieve. Single-tenant is not the same as single-user.
- **Ingestion pipeline scanning is a second line of defence**, not a substitute for retrieval-time filters. Injection markers can be encoded (base64, homoglyphs, zero-width characters) to evade scanners.
- **Cross-skill chaining**: if a retrieval site is both missing a tenant filter (cross-tenant leak) AND assembling content without a fence (indirect injection), file TWO chain_id-linked findings — one for `rag-pii-leak` and one for `rag-injection` — so the triage step and the report can see the compound risk.
- **Default severity is high** for confirmed multi-tenant + missing filter. Raise to critical when the architecture.md or codebase confirms financial records, health data, credentials, or regulated PII in the index. Lower to medium only when tenancy cannot be confirmed from static analysis.
- When in doubt about tenancy, classify as **Needs Manual Review** rather than Not Vulnerable — cross-tenant leakage is catastrophic when it fires and is almost always invisible in logs.
- Clean up intermediate files: delete `sast/ragleak-recon.md` and all `sast/ragleak-batch-*.md` files after `sast/ragleak-results.md` and `sast/ragleak-results.json` are written (Phase 3 step 6).
