Uncertainty-Guided Hypergraph Refinement (UGHR) methodology for medical image segmentation. Uses entropy-based uncertainty maps from coarse predictions to spatially guide targeted refinement in boundary/transition regions. Decouples foreground/background hyperedge prototypes to prevent noise propagation. Use when performing medical image segmentation with ambiguous boundaries, small lesions, or ill-defined edges.

# Uncertainty-Guided Hypergraph Refinement (UGHR)
## Problem
Lesions resemble surrounding tissues with ill-defined boundaries, causing unstable predictions in transition regions. Small-lesion cues get diluted by multi-scale feature extraction.
## Core Mechanism
### Entropy-Based Uncertainty Map
1. Generate coarse probability map from base segmentor
2. Compute pixel-wise entropy: H(p) = -sum(p_i * log(p_i))
3. High entropy regions = ambiguous boundaries requiring refinement
4. Low entropy regions = confident predictions, no refinement needed
### Decoupled Hypergraph Modeling
1. Split hyperedge prototypes into foreground and background groups
2. Build hypergraph over high-uncertainty regions only
3. Refine predictions by propagating information through foreground hyperedges
4. Suppress background noise by not connecting through background hyperedges
## When to Use
- Medical image segmentation with ambiguous boundaries
- Small lesion detection where cues are easily diluted
- Any segmentation task with tissue-similarity challenges
- Situations requiring targeted (not uniform) feature refinement
## Pitfalls
- Coarse prediction quality directly affects uncertainty map reliability
- Hypergraph construction cost scales with number of uncertain regions
- Background suppression may cause false negatives if thresholds too aggressive
- Requires careful calibration of entropy threshold for refinement trigger
## Verification
- Boundary Dice score should improve significantly over baseline
- Small lesion recall should increase without precision drop
- Uncertainty map should correlate with actual error regions
