# Check if bone mineral density measurement is indicated
## STEP 1 — Gather Information
Collect patient history: presence of osteoporosis risk factors (low body weight <60 kg, prior fragility fracture, chronic glucocorticoid use, hypogonadism, malabsorption, smoking, alcohol excess, family history of osteoporosis) and whether sex hormone therapy has been stopped after gonadectomy (orchiectomy, oophorectomy).
## STEP 2 — Rule In / Rule Out
If any osteoporosis risk factor is present OR sex hormone therapy has been discontinued after gonadectomy → proceed to Step 3; otherwise → BMD measurement not indicated.
## STEP 3 — Classify or Stratify
Classify as BMD indicated (no further stratification needed per guideline).
## STEP 4 — Decide
Order dual-energy X-ray absorptiometry (DXA) scan of lumbar spine and hip; repeat monitoring per clinical judgment or as per Table 7/9 in guideline (every 1–2 years if indicated).
## Clinical Guardrails / Mimics / Pitfalls
Do not order BMD solely based on transgender status without risk factors; interpret BMD using reference ranges for sex assigned at birth pending further data; avoid delaying BMD in patients on long-term hormone therapy who develop risk factors; ensure gonadectomy status is confirmed (surgical removal of gonads) before attributing indication to hormone cessation.
## Concrete Clinical Example
A 28-year-old transgender female underwent orchiectomy and stopped estrogen therapy 6 months ago; she has a BMI of 18 kg/m2 and reports a prior wrist fracture. Risk factors present → BMD DXA ordered shows T-score -2.1 at lumbar spine, leading to initiation of osteoporosis treatment.
