# Select statin based on renal clearance in chronic kidney disease
## STEP 1 — Gather Information
Collect eGFR, CKD stage, urine albumin-to-creatinine ratio, current statin use, LDL-C level, ASCVD risk, concomitant medications (especially cyclosporine, gemfibrozil), pregnancy status, age >75 years, and history of statin intolerance. Proceed to assess renal function and statin safety.
## STEP 2 — Rule In / Rule Out
Determine if the patient has CKD stages 1–4 or postrenal transplant (rule in) versus end-stage renal disease requiring dialysis (rule out). If CKD stages 1–4/postrenal transplant, proceed to statin selection; if ESRD on dialysis, consider statin continuation only if already on statin and do not initiate new statin therapy.
## STEP 3 — Classify or Stratify
Classify statins by renal clearance: prefer pitavastatin, pravastatin, or rosuvastatin (partial kidney clearance) over atorvastatin, fluvastatin, lovastatin, or simvastatin (hepatic clearance). Select a statin from the preferred list based on LDL-C goal and drug interactions.
## STEP 4 — Decide
Initiate the chosen statin at a starting dose appropriate for CKD stage (e.g., pravastatin 10–20 mg daily, rosuvastatin 5–10 mg daily, pitavastatin 1–2 mg daily); note that atorvastatin and fluvastatin do not require dose adjustment in CKD. Monitor for muscle symptoms, liver enzymes, and renal function, and reassess LDL-C and safety within 4–12 weeks.
## Clinical Guardrails / Mimics / Pitfalls
Avoid initiating statin therapy in pregnancy; do not use gemfibrozil with any statin due to increased rhabdomyolysis risk; do not rely solely on LDL-C targets without considering renal clearance and dose adjustment; avoid high-intensity statins in frail elderly or those with multiple interacting medications unless benefit clearly outweighs risk; do not assume all statins are safe in ESRD without nephrology input.
## Concrete Clinical Example
A 58-year-old with type 2 diabetes, CKD stage 3b (eGFR 38 mL/min/1.73m²), LDL-C 110 mg/dL, no ASCVD, and not on a statin requires lipid-lowering therapy. After confirming CKD stage 3b, a statin with partial kidney clearance is chosen; pravastatin 20 mg daily is initiated (dose adjusted for CKD), LDL-C rechecked in 6 weeks, and therapy continued if tolerated and LDL-C goal <70 mg/dL approached.
