# Interpret bone turnover marker change using least significant change
## STEP 1 — Gather Information
Collect two sequential measurements of the same bone turnover marker (e.g., serum total ALP, BSAP, P1NP, NTx, or CTx) obtained under comparable conditions (same assay platform, fasting status, time of day). Record the analytical coefficient of variation (CVa) and within‑subject biological variation (CVg) for the marker from laboratory validation data or published sources.
## STEP 2 — Rule In / Rule Out
Calculate the least significant change (LSC) using the formula LSC = 2.77 × √(CVa² + CVg²) (where 2.77 corresponds to a 95% confidence level for two measurements). If the absolute percent change between the two results exceeds the LSC, rule in a true biological change; otherwise, rule out that the observed difference is likely due to measurement variability.
## STEP 3 — Classify or Stratify
Express the change as a ratio to LSC (Δ/LSC). A ratio >1 indicates a statistically significant change; further classify direction: a decrease >LSC suggests treatment effect or disease attenuation, while an increase >LSC suggests disease progression or non‑adherence. If the ratio is ≤1, consider the change indeterminate and recommend repeat testing.
## STEP 4 — Decide
If a significant decrease is confirmed, continue current therapy and schedule next assessment per drug‑specific interval (e.g., 6–12 weeks for resorption markers, 2–3 months for formation markers). If a significant increase is observed, evaluate for treatment failure, non‑adherence, or complications and consider intensifying therapy or investigating alternative causes. If indeterminate, repeat the marker in 1–3 months before making therapeutic changes.
## Clinical Guardrails / Mimics / Pitfalls
Do not interpret changes that fall within the LSC as clinically meaningful; avoid making therapeutic decisions based on single‑marker fluctuations. Be aware of assay‑specific variability: total ALP has higher biological variation than specific markers (BSAP, P1NP, NTx) and may require larger changes to exceed LSC. Ensure samples are processed on the same automated platform to minimize analytical variation; urine markers show greater within‑subject variability than serum markers.
## Concrete Clinical Example
A patient with active Paget’s disease has serum NTx of 65 nM BCE at baseline and 48 nM BCE after 3 months of zoledronate. Laboratory CVa for NTx is 5% and CVg is 12%. LSC = 2.77 × √(0.05² + 0.12²) ≈ 2.77 × 0.13 ≈ 0.36 (36%). Percent change = (48‑65)/65 ×100 = ‑26%. Since |‑26%| < 36%, the change does not exceed LSC; thus the decrease is not statistically significant and treatment effect cannot be confirmed based on NTx alone.
