# Measure Plasma Renin and Aldosterone to Assess Mineralocorticoid Deficiency
## STEP 1 — Gather Information
Confirm PAI diagnosis (e.g., ACTH stimulation test). Obtain morning plasma renin activity (or concentration) and aldosterone level simultaneously. Document symptoms suggestive of mineralocorticoid deficiency (salt craving, postural hypotension, edema, hyponatremia, hyperkalemia) and review medications that affect RAAS (ACE inhibitors, ARBs, spironolactone, licorice, grapefruit juice, phenytoin).
## STEP 2 — Rule In / Rule Out
Is plasma renin above the upper limit of the reference range **and** serum aldosterone normal or low? If yes → rule in mineralocorticoid deficiency. If renin is not elevated or aldosterone is appropriately high → rule out mineralocorticoid deficiency due to PAI.
## STEP 3 — Classify or Stratify
If mineralocorticoid deficiency is confirmed, assess severity: marked renin elevation with low aldosterone and electrolyte abnormalities indicates greater need for replacement; mild renin rise may warrant lower fludrocortisone dose.
## STEP 4 — Decide
Initiate fludrocortisone replacement (starting dose 50–100 µg daily in adults) with normal salt intake. Monitor clinical response (symptoms, serum sodium/potassium, blood pressure) and adjust dose based on clinical assessment (salt craving, postural hypotension, edema).
## Clinical Guardrails / Mimics / Pitfalls
Do not interpret renin/aldosterone without excluding RAAS‑affecting drugs (ACEi, ARBs, NSAIDs, spironolactone, licorice, grapefruit juice, phenytoin). Do not apply to secondary adrenal insufficiency (renin typically low). Do not rely on aldosterone alone; an inappropriately normal/low aldosterone with elevated renin is required. Avoid fludrocortisone in patients with uncontrolled hypertension or volume overload. Do not restrict salt intake.
## Concrete Clinical Example
A 45‑year‑old woman with known PAI on glucocorticoid replacement reports fatigue, salt craving, and mild postural hypotension. Labs: Na 128 mmol/L, K 5.2 mmol/L. Plasma renin activity 35 ng/mL/hr (high), aldosterone 2 ng/dL (low‑normal). Simultaneous measurement shows elevated renin with inappropriately low aldosterone → mineralocorticoid deficiency confirmed. Start fludrocortisone 50 µg daily; after 2 weeks symptoms improve and electrolytes normalize.
