## STEP 1 — Gather Information

## STEP 2 — Rule In / Rule Out
Is the lesion suspected to be a head and neck paraganglioma based on imaging and clinical features? If yes, proceed to avoid fine needle cytology; if no, consider fine needle cytology for alternative diagnoses (e.g., thyroid nodule, lymph node metastasis).
## STEP 3 — Classify or Stratify
Subclassify the suspected head and neck paraganglioma as functional (elevated plasma-free metanephrines/normetanephrines or 3-methoxytyramine) or nonfunctional (normal biomarkers) to counsel on procedural risks.

Suspected nonfunctional head and neck paraganglioma. Per guideline, fine needle cytology is avoided; instead, SDHD genetic testing is ordered and surgical resection is planned after alpha-blockade.
